血管扩张剂治疗突发性聋的系统评价

摘　　要：目的:系统评价血管扩张剂治疗突发性聋的疗效和安全性。方法:运用系统评价的方法,计算机检索Cocharane图书馆,PubMed,EmBase,ISI,中国生物医学文献数据库,中国学术期刊全文数据库,VIP中文科技期刊全文数据库和万方数据库。纳入中外文血管扩张剂治疗突发性聋的随机对照试验(RCT)。按Cochrane系统评价标准评价纳入研究质量,对同质研究采用RevMan软件进行Meta分析,对不能进行Meta分析的数据,只进行描述性的定性分析。结果:共纳入28个RCT。Meta分析结果提示血管扩张剂与安慰剂对照的7个试验中,未发现血管扩张剂疗效优于安慰剂;血管扩张剂与血管扩张剂比较的14个试验中,未发现哪种血管扩张剂更有效;血管扩张剂与其他治疗对比的9个试验中,未发现血管扩张剂优于其他治疗方法。结论:血管扩张剂治疗突发性聋的疗效仍不清楚,由于上述的结论基本来自单个研究,研究质量参差不齐,大多数研究样本量小,因此使用结论时必须谨慎。目前尚无足够的证据证明哪种药物更有效,期待开展更多高质量、大样本、长期随访的RCT,以提供更可靠的证据。Objective：To evalute the efficacy and safety of vasodilators for sudden sensorineural hearing loss.Method：Based on the principles and methods of cocharne systematic reviews,we searched the cochrane central register of controlled trials,PubMed,Embase,ISI,the china biological medicine datebase,VIP,CNKI and wangfang database.Randomized controlled trials about using vasodilators to treat sudden sensorineural hearing loss were included.Meta-analysis was performed for the results of homogeneous studies using RevMan software.Result：Twenty eight randomized control trials met the inclusion criteria.Seven studies showed vasodilators was not more effective than placebo.From 14studies comparing vasodilators with vasodilators and 9studies comparing vasodilators with other drugs,no definite conclusion could be drawn.Conclusion：The evidence currently available does not support the use of vasodilators in the treatment of sudden sensorineural hearing loss.Further randomized,doubleblind,placebo-controlled trials are needed in order to define the efficacy and acceptability of vasodilators in the treatment of sudden sensorineural hearing loss.
