经尿道前列腺电切术联合耻骨上膀胱穿刺引流对重度良性前列腺增生的临床疗效分析

摘　　要：目的观察经尿道前列腺电切术(TURP)联合耻骨上膀胱穿刺引流术对重度良性前列腺增生的临床疗效。方法将94例重度良性前列腺增生患者随机分为研究组和对照组,每组各47例,分别接受TURP联合耻骨上膀胱穿刺引流术和单纯TURP术。对比观察两组前列腺切除大小、手术时间、留置导尿时间、持续冲洗时间、住院时间、排尿改善情况[国际前列腺症状评分(IPSS)和最大尿流率(Qmax)]和并发症。结果研究组手术时间、住院时间、留置导尿时间和持续冲洗时间均明显短于对照组,前列腺切除大小明显大于对照组,组间比较差异有显著性(P<0.05);两组术后IPSS评分和Qmax较术前均有明显改善,差异有显著性(P<0.05);组间比较研究组IPSS评分和Qmax虽优于对照组,但差异无显著性(P>0.05)。术后随访两组患者均无尿失禁和尿道狭窄发生。结论 TURP联合耻骨上膀胱穿刺引流术疗效好、手术创伤轻、并发症少,可推广应用于重度良性前列腺增生症患者。【Objective】To observe the clinical efficacy of transurethral resection of prostate（TURP） combined with suprapubic bladder puncture and drainage for the patients with severe severe benign prostatic hyperplasia（BPH）.【Methods】94 patients with severe BPH were randomly divided into study group and control group,47 cases in each group,they received TURP combined with suprapubic bladder puncture and drainage and simple TURP surgery respectively.The volume of resected prostate,operation time,catheterization time,continual washing time,hospitalization time,the improvement of urinating situation（IPSS score and maximum urinary flow rate） and complications of two groups were observed and compared.【Results】The operation time,hospitalization time,catheterization time and continual washing time in study group were significantly shorter than control group,the volume of resected prostate in study group were significantly more than control group,the differences were statistically significant（P0.05）;the postoperative IPSS score and Qmax were significantly improved compared with the preoperative ones in two groups,the difference was statistically significant（P0.05）;although the IPSS score and Qmax in study group were prior to that in control group when compared between the two groups,the difference was not statistically significant（P0.05）.There were no urinary incontinence and urethral stenosis occurred in patients of both groups in postoperative follow-up.【Conclusions】TURP combined with suprapubic bladder puncture and drainage has good efficacy,light surgical trauma and less complications,it can be applied to patients with severe BPH.
