摘  要:目的观察心房颤动(AF)射频消融术中肺静脉(PV)电隔离后自发电活动的表现形式。方法 60例AF患者在NavX导航下行环双侧PVs口部线性消融术至同侧PV电隔离。电隔离后环状标测电极及消融电极分别放置于同侧上下PV内,观察自发电活动,每侧10min。结果 31例患者(52%)观察到至少一根PV存在自发电活动。左侧发生比例显著高于右侧(35%vs.25%)(P<0.05)。偶发电活动显著高于持续电活动(47%vs.7%)(P<0.05)。结论 AF消融术中,PV电隔离后观察到偶发及连续两种形式自发电活动,显示了PV电位作为AF始动因素的重要意义。Objective To observe the automaticity electrical activity after isolation of pulmonary vein(PV) during atrial fibrillation(AF) ablation.Methods A total of 60 AF patients underwent circumferential pulmonary veins ablation to isolate the ipsilateral veins navigated by three dimensional mapping system.The circular mapping catheter and ablation catheter were put into the upper and lower PV,respectively,to record ten minutes of the autoelectrical activity after isolation.Results The autoelectrical activity in at least one pulmonary vein was recorded in 31 patients(52%),the rate of which was greater on the left than that on the right(35% vs. 25%)(P0.05).ConclusionTwo types of autoelectrical activity as regard to non-sustained and sustained automaticity were observed after PV isolation during AF ablation,indicating the important role of PV potential as a triggering factor in AF.
