局灶性磨玻璃影的周围型小肺癌的影像学与病理学对照研究

摘　　要：目的:探讨局灶性磨玻璃影(fGGO)的影像学表现在周围型小肺癌诊断中的临床价值。方法:回顾性分析15例手术及病理证实的直径≤2 cm的孤立性fGGO的周围型小肺癌,将其影像表现与病理诊断对照。结果:周围型小肺癌可以表现纯磨玻璃影或混合性磨玻璃影,磨玻璃影边界清楚,形态多不规则;周围型小肺癌fGGO中以细支气管型肺泡癌(BAC)和腺癌多见,胸膜凹陷征、分叶征、毛刺征是小肺癌fGGO的常见征象;周围型小肺癌表现为低量化fGGO时以腺癌多见,表现为高量化fGGO时以BAC多见。结论:周围型小肺癌fGGO的影像学表现具一定特征性,对早期肺癌的定性诊断具有一定的临床价值。Objective： To evaluate the imaging features of focal ground-glass opacity（fGGO）,in clinical diagnosis of small peripheral lung cancer.Methods： A retrospective study was carried out on 15 fGGO cases with spot diameters less than 2cm which were confirred as small peripheral lung cancer by pathological check.The radiological images were analysed and compared with pathologic findings.Results： Small peripheral lung cancer could be represented as pure ground-glass opacity or mixed ground-glass opacity on high resolution computeriged tomography（HRCT）.The edge of ground-glass shadow was clear,and mostly was irregular in shape;The small peripheral lung cancer displayed fGGO were mostly BAC or adenocarcinoma,and the signs of pleural indentation,lobulation and burr were often showed in fGGO of small lung cancer;When displayed as low-proportion fGGO,the small peripheral lung cancer was mostly adenocarcinoma,while those characterized with high-proportion fGGO were mostly BAC.Conclusion： The imaging finding of fGGO in small peripheral lung cancer is characteristic,and is useful for clinical diagnosis of lung cancer in early stage.
