首次透析负责制管理模式对血液净化中心护理执行力的影响

摘　　要：目的探讨首次透析负责制管理模式对血液净化中心护理执行力的影响。方法制定首次透析负责制的内容、执行流程、培训计划和管理制度,对103例血液透析患者实施首次透析负责制管理,采用明尼苏达满意问卷评价护理人员工作满意度,评价透析患者的透析充分性和满意度。结果在透析充分性的5个项目中,尿素清除指数、蛋白分解率、血压、血清白蛋白4个项目与实施首次透析负责制管理前比较,差异有统计学意义(P<0.01),血红蛋白比较差异无统计学意义;患者满意度从管理前的93.5%上升至98.9%。在工作满意度调查问卷的20个条目中,内在满意度维度中有8个条目有统计学意义,外在满意度维度中有3个条目有统计学意义(P<0.05或P<0.01)。结论实施首次透析负责制管理模式,可提高血液净化中心的护理执行力,提高护理人员工作满意度,提高透析患者的满意度和透析充分性。Objective To explore the effect of first hemodialysis responsibility on execution of blood purification center.Methods The content,process,training plan and management system of first hemodialysis responsibility were worked out and applied among 103 hemodialysis patients.The nursing job satisfaction was evaluated with MSQ.The dialysis adequacy and satisfaction of dialysis patients were evaluated as well.Results There were significant differences of the urea clearance index,PCR,blood pressure,serum albumin,and patient satisfaction and nursing job satisfaction over the implementation of management（P 0.01 or P 0.05）.Conclusion The first hemodialysis responsibility can improve care execution,improve nursing job satisfaction and the satisfaction of dialysis patients and dialysis adequacy.
