摘 要:目的通过测量人工气道患者不同体位时的气囊压力,为患者采取合适的体位、适宜的气囊压力提供科学临床依据。方法 2008年1—10月在北京市某三级甲等医院老年病房行气管切开和气管插管的患者共51例,采用自身对照设计方案,应用气囊压力表于呼气末分别测量患者在平卧位、半卧位(床头抬高45°)、右侧卧位、左侧卧位时气管导管的气囊内压力,并进行比较。结果不同体位气囊压力由低到高依次为:半卧位(23.0±7.6)cmH2O、平卧位(25.0±9.1)cmH2O、左侧卧位(28.0±9.0)cmH2O、右侧卧位(28.0±9.9)cmH2O。半卧位时气囊压力显著低于平卧位及左、右侧卧位时的气囊压力(P<0.01或P<0.001)。结论人工气道患者尽量采取半卧位,以减轻气囊压力对气管黏膜的影响,减少相关并发症的发生。Objective To measure and compare cuff pressure of cannula in different body positions for the patients with artificial airway and to provide clinical evidence in choosing suitable body position and cuff pressure.Methods The cuff pressure of cannula in different body positions(horizontal position,semireclining position,right lateral position and left lateral position) were measured by a cannula manometer.Differences of pressure among the positions were compared.Results The cuff pressure of different positions from low to high were semireclining position(23.0±7.6) cmH2O,horizontal position(25.0±9.1) cmH2O,left lateral position(28.0±9.0) cmH2O and right lateral position(28.0±9.9) cmH2O.The pressure of semireclining position was significantly lower than those of horizontal position and left/right lateral position(P0.01 of P0.001).Conclusion The semireclining position should be widely used,so as to improve the nursing quality of the artificial airway and avoid related complications.
