立止血在69例脾切除贲门周围血管断流术围术期的应用观察

摘　　要：目的研究立止血在脾切除贲门周围血管断流术围手术期的止血作用。方法随机将患者分为治疗组(35例)和对照组(34例),治疗组术前、术中、术后给予立止血,对照组在相同时期给予常规止血药物。比较术中出血量、术后静脉血D-二聚体(D-D)以及术后血性引流液量的变化。结果治疗组术中出血量为(329.1±31.5)m l,对照组为(538.5±28.6)m l(P<0.05);治疗组术后静脉血D-D为(0.7912±0.069)ng/L,对照组为(1.1831±0.074)ng/L(P<0.05);血性引流液量在治疗组为(326±31)m l,对照组为(557±19)m l(P<0.05)。结论在脾切除贲门周围血管断流术围手术期应用立止血的止血作用明显优于一般常规止血药物。Objective To study the hemostatic effect of reptilase on Splenectomy With Periesophagogastric devascularization in perioperative period.Methods The study was conducted in 69 patient.They were randomly divided into 2 groups：reptilase group（n=35） and control group（n=34） In reptilase group,the reptilase was given in pre-operative,post-perative and intraoperative period and in control group,other commonly-used hemostants were given in the same way as in reptilase group.The amount of bleeding,the content of D-D in venous blood,the quantity of postoperative bloody drainage were compared between 2 group.Results The amount of intraoperative bleeding,postoperative contents of D-D in venous blood and the quantity of bloody drainage in reptilase and control group were（329.1±31.5）ml,（538.5±21.6）ml（P〈0.05）;（0.7912±0.069）ng/ml,（1.1831±0.074）ng/ml（P〈0.05） and（326±31）ml,（557±19）ml,respectively.Conclusion The hemostatic effect of reptilase in Splenectomy With Periesophagogastric devascularization in perioperative period is obviously better.
