摘　　要：目的:探讨早期腹膜透析联合血液滤过治疗在急性重症胰腺炎中的应用价值。方法:将急性重症胰腺炎患者40例随机分为A组和B组,A组患者在常规治疗基础上在入院24h内给予腹膜透析联合血液滤过治疗,B组患者在常规治疗基础上在入院24h后给予腹膜透析联合血液滤过治疗,连续治疗7d。比较两组患者的腹痛、腹胀缓解时间、血淀粉酶恢复正常时间、WBC恢复正常时间以及感染率、病死率、并发症发生率、住院时间和住院费用。结果:A组患者腹痛、腹胀缓解时间、血淀粉酶恢复正常时间、WBC恢复正常时间、住院时间较B组明显缩短,感染率、病死率、并发症发生率和住院费用较B组明显降低。结论:早期应用腹膜透析联合血液滤过治疗急性重症胰腺炎效果明显,可较早缓解症状并缩短病程、减低感染率、病死率、并发症发生率、住院时间及住院费用,且越早运用效果越明显。Objectives： To evaluate the efficacy of Combination of peritoneal and dialysis （PD） hemofiltration （HF） in the early treatment of severe acute pancreatitis. Methods： 40 patients with SAP were randomlv divided into two groups, group A and group B, Both groups were treated by the conventional mode of therapy. Patients in the group A were treated with PD and H F in 24 hours for seven days, Patients in the group B were treated with PD and HF afer 24 hours for seven days. The release time of abodominal pain and distention , the recovery time of serum amylase and white blood cel1 count （WBC）.the time of hospital stay, cost of treatment in hospital, the infection rate,the death rate and the complications rate of two groups were compared. Results： The release time of abodominal pain and distention , the recovery time of serum amylase and white blood cel1 count（WBC） of group A were significantly shorter than that of group B.the time of hospital stay, cost of treatment in hospital , the infection rate,the death rate and the complications rate of group A were significantly shorter than that of group B. Conclusions： The effect of combination of peritoneal dialysis and hemofiltration early treatment on severe acute pancreatitis was significant,it can on remising syndrome, shortening course of treatment, reduce the infection rate, the death rate,the complication rate, length of stay and cost of hospitalization.The earlier be used,the better to patients.
