摘 要:目的:应用多平面经食管体元模型动态三维超声重建及三维提取技术评价正常人和二尖瓣脱垂者二尖瓣环的立体形态,为二尖瓣环成形术的合理设计提供依据。方法:研究对象包括19例正常人和21例经手术证实的二尖瓣脱垂伴关闭不全患者。结果与结论:无论正常人和二尖瓣脱垂者,其瓣环在收缩期均为一非平面的“马鞍”形,前后侧较高,左右交界侧较低。正常组二尖瓣环前、后叶间夹角(a)为145.89±13.18度,脱垂组145.12±15.79度;正常组后叶侧置于零度位时前叶顶部与底边的高度(b)为1.27±0.41cm,脱垂组为1.30±0.26cm;正常组前后叶侧顶点连线至内外交界连线的距离(c)为1.08±0.24cm,脱垂组为1.10±0.19cm。正常组和二尖瓣脱垂组a、b、c差别无显著意义(P>0.05。Objective:To evaluate morphology of mitral annulus,the multiplane transesophageal dynamic three dimensional echocardiographic reconstruction and extract technique were used for the rational design of prosthetic mitral annulus as well as improving effects of mitral annuloplasty.Methods:We studied 19 normal cases and 21 patients with mitral prolapse identified by surgery.Results and Conclusion:The 'saddle'shape of mitral annulus was demonstrated both in the normal subjects and patients with mitral prolapse.The anterior and posterior attachments of mitral annulus were higher than medial and lateral attachments.The angle(α)between anterior and posterior attachments in normal group was 145 89±13 18°,and 145 12±15 79° in prolapse group.The height(b)between anterior attachment of mitral annulus and basic line with posterior leaflet at 0 ° position in normal group was 1 27±0 41cm,and 1 30±0 26 cm in prolapse group.The distance(c)from the line of two top points between anterior and posterior attachment to the line of two junctions between medial and lateral attachment was 1 08±0 24cm in normal group,and 1 10±0 19cm in prolapse group.There were no significant differences between two groups( P >0 05).
