皮瓣移植修复组织缺损临床分析

摘　　要：目的分析研究皮瓣移植修复组织缺损的临床效果。方法应用１３种类型的皮瓣移植修复组织缺损１３３例，皮瓣类型有肩胛皮瓣１１例，股前外侧皮瓣２６例，侧腹部皮瓣１１例，足背皮瓣９例，前臂皮瓣１３例，腓肠神经营养血管皮瓣１０例，下腹部皮瓣３１例，还有小腿内侧皮瓣、小腿外侧皮瓣、腓肠肌皮瓣、背阔肌皮瓣及阔筋膜张肌皮瓣等２２例。其中吻合血管皮瓣游离移植修复６２例，带血管蒂岛状皮瓣移植修复７１例。结果有２例失败，有５例皮瓣部分坏死，其余皮瓣均全部成活，成功率为９６．２％，修复的效果良好。结论皮瓣移植修复组织缺损，皮瓣色泽、质地良好，只要严格掌握手术适应证。Objective To summarize and analyse the effect of repairing the soft tissue defect with transplantation flap Method Thirteen different kinds of flaps or musculocutaneous flaps were transplantated for 133 cases of soft tissue defect These types include the scapular flap (11 cases), the anteriolateral femoral flaps (26 cases), the side abdominal flaps(11 cases), the flap of the dorsum of the foot (9 cases), the forearm flaps (13 cases), the posterior crural flap with sural nerves (10 cases), the hypogastric flap(31 csses), and other 22 cases with the medial crual flap, the lateral crural flap, the gastrocnemius flap, the tensor fasciae latae flaps and the latissimus dorsi flaps There were 62 transplantation cases by vessels anastomosed free skin flap and 71 cases by vessels pedicled flaps Result There were partally survived in 5 of the 133 cases, and necrosed in 2 cases, and completely survived in the other 126 cases The rate of survival was 96 2% The effect of repair was good Conclusion Repairing skin tissue defect with transplantated flap has some advantages: colour and texture of flaps was excellent In order to achieve the good desired results, it is important that grasp the surgical indication of varionus flaps and keys for operation
