摘　　要：目的了解慢性乙型肝炎患者血清、肝内HBV DNA定量及肝组织HBV DNA载量与肝组织损害程度的关系。方法经血清学及肝穿刺病理证实为慢性乙型肝炎患者38例,分为2组(HBeAg阳性组及HBeAg阴性组)。采用ELISA法测定HBV-M、实时荧光定量检测血清及肝内HBV DNA定量、肝组织HBV DNA载量。结果 HBeAg阳性组血清、肝内HBV DNA定量及肝组织HBV DNA载量均高于HBeAg阴性组(P均<0.001);血清HBV DNA定量随肝脏纤维化程度加重而升高(P<0.01),但与肝脏炎症程度未见明显相关;肝组织HBV DNA载量则随肝脏炎症及纤维化程度加重而降低(P<0.001)。结论血清HBV DNA定量与肝组织纤维化程度呈正相关,肝组织HBV DNA载量则与肝组织的炎症及纤维化程度呈明显负相关。Objective To understand the correlation of serum,intrahepatic HBV DNA quantity and liver tissue HBV DNA loads and injury severity in liver tissue of chronic hepatitis B.Methods Liver specimens were taken from 38 chronic hepatitis B（CHB） patients and divided into two parts,one was evaluated for hepatic inflammation and fibrosis degrees by histological examination,the other for intrahepatic HBV DNA quantity and liver tissue HBV DNA loads were detected by PCR-fluorescence.Serum HBV DNA quantity was assessed simultaneously.Patients were classified into two groups： HBeAg positive and HBeAg negative.Results Serum,intrahepatic HBV DNA quantity and liver tissue HBV DNA loads in HBeAg positive CHB patients were all higher than those in HBeAg negative CHB patients（P 0.001 without exception）.Serum HBV DNA quantity increased with aggravation of hepatic fibrosis（P 0.001）,while liver tissue HBV DNA loads decreased with aggravation of hepatic inflammation and fibrosis（P 0.001）.Conclusion Serum HBV DNA quantity is positively correlated to the degree of hepatic fibrosis;liver tissue HBV DNA loads is negatively correlated to hepatic inflammation and fibrosis degree.
