摘  要:目的研究不同甲状腺功能状态下血脂的特点及甲状腺功能与血脂的关系。方法收集甲亢、甲减、亚临床甲亢、亚临床甲减和正常甲功者共568例,分析各组血脂谱的差别,并应用相关及多重回归分析影响这些指标的因素。结果甲亢时TC、TG、HDL、LDL、ApoB降低;而甲减时TC、LDL、ApoB升高,HDL降低;亚临床甲减较亚临床甲亢对代谢影响更大,可出现TC、LDL、ApoB升高;而亚临床甲亢组上述指标均无明显变化。TSH对甲亢组TC、LDL、ApoB,甲减组TC和亚甲减组血脂谱影响较大。结论甲状腺功能状态与血脂等代谢指标相关,且TSH与血脂异常关系密切。Objective To explore the characteristic of lipid metabolism under different thyroid functions.Method We collected 568 individuals with hyperthyroidism,hypothyroidism,subclinical hyperthyroidism,subclinical hypothyroidism and euthyroidism to analyze the difference in serum lipid between each group,and identify determinants using correlation analysis and multiple linear regression analysis.Results TC,TG,HDL,LDL,and ApoB were reduced in the case of hyperthyroidism while TC,LDL and ApoB were raised and HDL lowered under hypothyroidism.Patients with subclinical hypothyroidism suffered from more abnormal metabolism than those with subclinical hyperthyroidism and showed higher TC,LDL and ApoB and UA which were unchanged in subclinical hyperthyroidism patients.TSH had great impact on TC,LDL and ApoB in hyperthyroid group on TC in hypothyroid group and on serum lipid spectum in subclinical hypothyroid group.Conclusion Thyroid function is closely correlated with lipid metabolism.
