中药鼻腔冲洗对术后鼻窦炎康复的促进作用

摘　　要：目的观察中药溶液鼻腔冲洗对慢性鼻窦炎术后康复过程的影响。方法常规鼻内镜手术治疗的Ⅱ型3期慢性鼻窦炎鼻息肉患者56例,随机分为两组,每组28例,均同法予以围手术期综合治疗。术后第3天开始,实验组应用中药溶液进行鼻腔冲洗,对照组则用生理盐水同法冲洗,1次/d,连续1周。分别于术后2、4周观察术腔黏膜恢复情况,并测试鼻黏膜纤毛传输速率。术后随访6个月,比较两组疗效。结果术后2周时,2组动物均见术腔黏膜肿胀,有少量结痂和少量分泌物,鼻腔纤毛传输速率无明显组间差异;术后4周时,实验组术腔病变明显减轻,鼻黏膜纤毛传输速率加快。随访结束时,实验组治疗有效率明显高于对照组。结论中药溶液鼻腔冲洗能够有效促进鼻内镜手术后的术腔康复过程。Objective To observe the promoting effect of postoperative nasal irrigation with herbal medicine solution on the healing course of mucosa in operative cavity following endoscopic sinus surger （ESS） based on a clinical trial. Methods Included in this study were 56 cases with chronic sinusitis and/or nasal polyps at type II and stage 3 treated by routine ESS in our Hospital. They were randomly divided into 2 groups, with 28 in treating group （TG） and 28 in controlling one （CG）, all given the same combined perioperative therapy. The cases in TG received the postoperative nasal irrigation procedure with herbal medicine solution, begun at the 3rd postoperative day, once each day and lasted for one week, and those in CG were also given the procedure in the same way but with normal saline instead of herbal medicine solution. Then, careful observation was made on the healing status of mucosa in operative cavity in the postoperative 2nd and 4th weeks respectively, with the mucociliary transport rate （MTR） determined at the same time. Furthermore, a period of following up lasted for 6 months was carried out among all these patients to analysis the difference in their therapeutic effect between the two groups. Results In the 2nd week of postoperative course, swollen mucosa with a small amount of dry scab and secretion could be found in the operative cavity among all the cases, with no significant difference in MTR between the two groups. In contrast, obviously reduced swollen degree of nucosa, the amount of dry scab and secretion were seen in the operative sinous cavity of cases in TG, with significantly speeded up MTR, when compared with that found in CG in the 4th postoperative week. By the end of following up period, it was found that the difference in the total effetcive rates between the two groups was statistically significant, with much higher effeteive rate in TG than that in CG.
