摘　　要：目的探讨鼻咽癌侵犯翼腭窝的MRI表现特点。方法对46例具完整的临床和MRI资料并经病理活检证实的鼻咽癌翼腭窝受侵者行回顾性分析。结果 46例中有53侧受侵,翼腭窝受侵的MRI表现为:正常脂肪信号被病灶取代46例;翼腭窝扩大33例;翼腭窝内异常强化肿块影23例;翼腭窝周壁骨质破坏21例。邻近组织结构受累情况:20例合并海绵窦破坏,28例侵犯蝶窦,27例侵犯破裂孔,5例卵圆孔受累。结论鼻咽癌侵犯翼腭窝具有特征的MRI表现,MRI能判断其侵犯和扩散的途径及邻近组织结构受累情况。Objective To explore the MRI features of pterygopalatine fossa（PPF） invasion of nasopharyngeal carcinoma（NPC）.Methods Forty-six cases of pathologically proved NPC with PPF involvement,who were with whole clinical information and MRI images,were analyzed retrospectively.Results Fifty-three PPF invasion lesions could be found in the forty-six NPC cases.The common MRI features of NPC with PPF invasion were as follows：the disappearance of normal fat signal in the PPF in 46 cases,the enlargement of PPF in 33 casesL？abnormal enhanced masses in PPF in 23 cases and bone structure destruction of PPF in 21 cases.Adjacent structure involvements were as follows：cavernous sinus invasion in 20 cases,sphenoid sinus invasion in 28 cases,foramen lacerum involvement in 27 cases,foramen involvement in 5 case.Conclusion MRI can show characterized apperance for PPF involvement of NPC patients and provide its invading and spreading route and invasion of its surrounding structure.
