摘  要:目的:比较腰椎骨折AF椎弓根内固定系统固定结合经伤椎椎弓根椎体内植骨与后外侧植骨两种方法治疗腰椎压缩骨折疗效的优劣,为临床选择提供依据。方法:回顾性分析2006年7月~2009年10月沧州市中心医院脊柱外科收治的64例腰椎骨折患者临床资料,根据影像学检查,比较经伤椎椎弓根椎体内植骨(实验组)和经后外侧植骨(对照组)在椎体高度的变化、Cobb角的矫正程度及临床症状等方面的差异性。结果:实验组与对照组患者均获得随访,经病椎椎弓根行椎体内植骨内固定组在改善椎体高度、椎体后凸角的矫正程度、临床症状改善方面均明显优于对照组(P<0.05)。结论:AF椎弓根内固定系统固定结合经伤椎椎弓根行椎体内植骨治疗腰椎压缩骨折在减少迟发性腰背痛、神经症状出现、矫正度丢失、内固定松动断裂等并发症方面有明显优势。Objective:To compare the efficacy of transpedicular morselized bone grafting and posterolateral grafting when they were used together with AF spinal fixation system to treat vertebral compression fractures.Methods:A retrospective analysis was conducted on 64 patients with vertebral compression fractures that were admitted to Cangzhou Central Hospital during July 2006 October 2009.Image data of patients that underwent transpedicular morselized bone grafting(experimental group)and posterolateral grafting(control group)together with AF spinal fixation system were analyzed.Changes in vertebral body height,Cobb angle correction levels and clinical symptoms after surgery were compared.Results:Outcomes including changes in vertebral body height,Cobb angle correction levels and improvement of clinical symptoms of the experimental group were significant better than that of the control group(P〈0.05).Conclusion:Transpedicular morselized bone grafting used together with AF spinal fixation system in patients with vertebral compression fractures shows advantages in reducing complications like delayed back pain,neurological symptoms,loss of correction,breakage of internal fixation.
