摘 要:目的:探讨早期无创正压通气在创伤性湿肺患者中的疗效。方法:采用双水平气道内正压(Bi-PAP)无创通气治疗早期创伤性湿肺患者,动态观察患者动脉血气分析和呼吸频率,并与治疗前相比较。结果:采用BiPAP无创通气治疗后,较前面罩吸氧,pH值和PaCO2无明显变化(P>0.05),PaO2和呼吸频率明显改善(P<0.05),未见明显不良反应。结论:早期BiPAP无创通气能有效改善创伤性湿肺患者的氧合,是早期治疗创伤性湿肺的有效方法,但是一旦病情进展,应及时采取有创机械通气。Objective:To explore the effect of non-invasive ventilation at early stage on traumatic wet lung.Methods:Non-invasive ventilation with bi-level positive airway pressure(BiPAP)was carried out to treat patients with traumatic wet lung.With arterial blood gas analysis and breathing frequency measurement,all patients were under dynamic monitoring,and the therapeutical comparison was carried out.Results:After treatment,there was no significant change in pH and PaCO2 comparing with those of patients with oxygen mask(P〉0.05),but remaked improvement in PaO2 and breathing frequency(P〈0.05).Besides,no side effect occurred.Conclusions:Non-invasive ventilation at early stage can effectively improve oxygenation status in patients with traumatic wet lung,and is effective treatment.However,at the progressive stage,invasive mechanism ventilation is preferred.
