摘  要:目的通过检测系统性红斑狼疮(SLE)患者血清中高迁移率族蛋白1(HMGB1)来探讨SLE的发病机制。方法随机选择SLE患者60例,根据SLE疾病活动指数(SLEDAI)评分,分为活动组(≥10分)和稳定组(<10分)各30例,随机选择健康志愿者20名作为健康对照组。采用酶联免疫吸附试验(ELISA)法检测各组血清中HMGB1水平,并分析其与SLE临床表现及其他实验室指标之间的相关性。结果活动期SLE患者HMGB1血清水平较稳定期与健康对照组的表达水平呈明显升高,差异有统计学意义(P<0.01);而稳定组与健康对照组的差异无统计学意义(P>0.05)。血清HMGB1水平与SLEDAI、尿蛋白定量(24h)水平、抗Clq抗体、抗核小体抗体成正相关(P<0.05),与C反应蛋白成负相关(P<0.05);在抗双链DNA(dsDNA)抗体阴性组与阳性组,HMGB1的含量差异有统计学意义(P<0.05)。结论活动期SLE患者血清HMGB1水平升高,且与疾病活动指标具有相关性,提示HMGB1可能参与SLE的发病。Objective To investigate the relationship between high mobility group box protein 1(HMGB1)and pathogenesis of systemic lupus erythematosus(SLE).Methods According to the SLE activity index(SLEDAI),60 SLE patients were divided in two groups averagely(activity group:SLEDAI≥10;stable group:SLEDAI10).The content of HMGB1 in serum of the 60 SLE patients and 20 healthy people(as control) were detected with ELISA assay,and the correlation between HMGB1 and other index were analyzed statistically.Results The content of HMGB1 in serum of activity group patients were significantly higher than that in stable group and healthy controls(P〈0.01),but there was no difference between stable group and healthy controls(P〉0.05).The content of HMGB1 in serum were positively correlated with SLEDAI,urine protein(24 h),anti-C1q antibody,anti-nucleosome antibody(P〈0.05),but negatively correlated with C-reaction protein(P〈0.05).The content of HMGB1 in anti-dsDNA antibody negative group and in anti-dsDNA antibody positive group were different statistically(P〈0.05).Conclusion The content of HMGB1 elevated in active group,and it was correlated with SLEDAI,which indicate that HMGB1 may play an important role in pathogenesis of SLE.
