摘 要:目的探讨乙型肝炎病毒(HBV)不同感染谱患者外周血人类白细胞抗原(HLA)-A2限制性HBV核心抗原(HBcAg)特异性细胞毒性T淋巴细胞(CTL)的功能。方法将HBV感染者分为显性感染组、隐性感染组、无症状携带组、健康对照组,用HBcAg18-27V和HBcAg18-27I2条特异性多肽刺激各组外周血单核细胞(PBMC)后,酶联免疫斑点(ELISPOT)检测抗原表位特异性CTL分泌干扰素(IFN)-γ的应答比例和平均反应强度。结果各组对2条多肽的应答比例和平均反应强度差异均无统计学意义(P>0.05);隐性组的应答比例和平均反应强度均明显高于显性组、携带组和对照组(P<0.05);携带组应答比例和平均反应强度与对照组差异无统计学意义(P>0.05)。结论 HBV不同感染谱患者外周血特异性CTL对HBcAgl8-27V/I刺激应答水平有差别,隐性感染者有较高水平特异性CTL应答。Objective To study the function of HLA-A2 restricted and Hepatitis B core antigen (HBcAg) specific cytotoxic T lymphocytes (CTL) in peripheral blood of patients with various spectrums of Hepatitis B virus (HBV) infection. Methods HBV patients were divided into apparent infection group, inapparent infection group, asymptomatic carrier group and control group. After stimulating peripheral blood mononuclear cells (PBMC) in each group with HBcAg18-27V and HBcAg18-27I these two specific polypeptides, enzyme-linked immunospot assay (ELISPOT) was used to detect the response ratio and mean response intensity of specific CTLs' secreting IFN-γ. Results In each group, there was no statistical difference of response ratios and mean response intensity to HBcAg18-27V and HBcAg18-27I (P〉0.05), response ratio and mean response intensity in inapparent infection group were obviously higher than those of apparent group, asymptomatic carrier group and control group (P〈0.05), there was no statistical difference of response ratio and mean response intensity between carrier group and control group (P〉0.05). Conclusion Responses of specific CTLs in peripheral blood of patients with different HBV infection spectrums to HBcAg18-27V and HBcAg18-27I are various, and those with inapparent infection might be with a higher specific CTL response.
