右旋糖酐铁注射液致腰腹部严重坠痛感1例

摘　　要：1例42a女性,因慢性肾炎综合征,伴肾性贫血,将右旋糖酐铁注射液25mg溶于0.9%氯化钠注射液100mL中静脉滴注。用药后约2min患者出现胸闷,5min后出现腰部及腹部剧烈坠痛感,全身呈屈曲样,神志清楚,T37.2℃,P88次·min-1。立即停药,给予托拉塞米注射液20mg、地塞米松注射液5mg等对症治疗,约5min后,患者症状明显缓解。One 42-year-old female patient with chronic nephritic syndrome accompanied with renal anemia was intravenously administered iron dextran injection 25 mg dissolved in 100 mL of 0.9% sodium chloride injection.About two minutes after the infusion,the patient developed chest distress,and five minutes later she presented serious bearing-down pain in lumbus and abdomen,body flexion and conscious mind,with temperature of 37.2 ℃ and pulse rate of 88 beats per minute.The drug was discontinued immediately,and torasemide injection 20 mg,dexamethasone injection 5 mg were administered for symptomatic treatment.Five minutes later,the symptom relieved obviously.
