宫颈癌患者凝血功能的检测及意义

摘　　要：目的研究宫颈癌患者止凝血功能指针的变化,了解宫颈癌患者凝血功能改变的意义。方法将78例宫颈癌患者按病理分期分为Ⅰ～Ⅱ期组(49例)、Ⅲ～Ⅳ期组(29例),采用SYSMEXCA-7000全自动血凝分析仪测定不同分期的宫颈癌患者和62例宫颈良性病变患者的凝血酶原时间(PT)、凝血酶时间(TT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB)含量及D-二聚体(D-dimer)水平。结果宫颈癌患者的FIB、D-dimer水平与健康体检者和宫颈良性病变患者相比均显著增高,差异有统计学意义(P<0.01),而PT、TT、APTT水平差异无统计学意义(P>0.05)。肿瘤的恶性程度分期为Ⅲ～Ⅳ期患者血浆FIB、D-dimer水平均高于Ⅰ～Ⅱ期的患者,差异有统计学意义(P<0.05),而PT、TT、APTT在两组间差异无统计学意义(P>0.05)。结论宫颈癌患者常伴随凝血及纤溶功能异常改变,监测患者体内FIB、D-dimer的改变可作为预防血栓形成及慢性弥漫性血管内凝血等凝血性疾病的有效措施,且能为宫颈癌的病情进展与临床分期提供帮助。Objective To explore the changes of blood coagulation indexes in the patients with cervical cancer for understanding their clinical significance.Methods PT、TT、APTT、FIB、D-dimer levels of 62 patients with benign cervical lesions and 78 cancer cervical patients including 49 pathological term ⅠtoⅡpatients and 29 pathological termⅢtoⅣpatients were detected by SYSMEX CA-7000.Results The level of FIB、D-dimer in the patients with cervical cancer were significantly higher than that in the patients with benign cervical lesions and physical examinees（P〈0.01） ,while there were no differences of PT、TT and APTT levels between the patients with cervical cancer and those in other two groups（P〉0.05） .In different period of cervical cancer,the level of plasma FIB、D-dimer of term ⅢtoⅣ were higher than that in termⅠtoⅡ（P〈0.05） ,while there were no significant differences of the level of PT、TT and APTT in the two groups（P〉0.05） .Conclusion The patients with cervical cancer are usually accompa-nied with dysfunction of blood coagulation and fibrinolytic systems,so monitoring their alterations in FIB、D-dimer levels could be a valid measure to prevent coagulation disorder and also useful for the diagnosis and clinical stage of cervical cancer.
