摘 要:目的观察类风湿关节炎(RA)患者外周血CD4+CD2+5调节性T细胞比例及血清白介素17(IL-17)、转化生长因子β(TGF-β)水平变化及三者的相互关系。方法选取33例RA患者,其中RA活动期患者22例,RA非活动期患者11例。另选取15例健康体检者作为对照组。RA患者及健康者外周血CD4+CD2+5调节性T细胞比例采用流式细胞仪(FCM)检测,血清IL-17、TGF-β水平采用双抗体夹心酶联免疫吸附(ELISA)法检测。结果 RA活动期患者外周血CD4+CD2+5调节性T细胞比例〔(1.46±0.72)%〕显著低于对照组〔(2.72±2.07)%〕,血清IL-17〔(384±106)pg/ml〕、TGF-β〔(23.8±10.0)pg/ml〕水平显著高于RA非活动期患者〔(245±110)pg/ml与(14.7±7.5)pg/ml〕和对照组〔(174±50)pg/ml与(9.8±4.0)pg/ml〕,差异均有统计学意义(P<0.01)。血清IL-17水平与外周血CD4+CD2+5调节性T细胞比例呈负相关(r=-0.375,P<0.05),与血清TGF-β水平呈正相关(r=0.650,P<0.01)。结论外周血CD4+CD2+5调节性T细胞比例及血清IL-17、TGF-β表达水平与RA疾病活动有关,且三者相互联系,相互影响,相互制约,共同参与RA的发生、发展。Objective To explore the changes and relationship of CD+4CD+25 regulatory T cells(Tregs),interleukin-17(IL-17),transforming growth factor beta(TGF-β) in patients with rheumatoid arthritis(RA) by detecting the levels of CD+4CD+25 Tregs,IL-17,TGF-β in the peripheral blood of patients with active and inactive RA.Methods 33 RA patients were selected,22 with active RA and 11 with inactive RA.15 healthy people were selected as control group.CD+4CD+25 regulatory T cells in peripheral blood of RA patients and healthy people was detected by FCM,while the levels of IL-17,TGF-β were detected by ELISA.Results The rate of CD+4CD+25 regulatory T cells in peripheral blood of RA patients〔(1.46±0.72)%〕 was markedly lower than that in control group〔(2.72±2.07)%〕.The levels of IL-17〔(384±106)pg/ml〕and TGF-β〔(23.8±10.0)pg/ml〕 of PA patients were markedly higher than that of inactive RA patients and control group,which were 〔(245±110)pg/ml,(14.7±7.5)pg/ml〕 and 〔(174±50)pg/ml,(9.8±4.0)pg/ml〕 respectively,the difference was significant(P0.01).The level of IL-17 was correlated negatively with CD+4CD+25 regulatory T cells(r=-0.375,P0.05),while it was correlated positively with TGF-β(r=0.650,P0.01).Conclusion RA is closely related to CD+4CD+25 regulatory T cells in peripheral blood and the levels of IL-17 and TGF-β,which interact with each other and were all involved in the occurrence and development of RA.
