血浆冷沉淀治疗多发伤和手术创伤的临床观察

摘　　要：目的观察对32例多发伤和手术创伤病人早期输注血浆冷沉淀的临床效果。方法对32例ISS90评分平均为（18．21±5．63）分的多发伤以及手术创伤病人输注血浆冷沉淀共580单位（平均18．1单位）；随机对12例病人在输注冷沉淀前、后行血常规、出血时间及纤维结合蛋白含量测定；并对其中6例多发伤患者在输冷沉淀前、后0．5、2、24小时测定血浆Fn水平。结果患者输注血浆冷沉淀后与输注前相比，出血时间明显缩短（P＜0．01）；血浆Fn水平在输注后0．5小时显著提高（P＜0．05），2小时后仍维持在较高水平。结论对多发伤和手术创伤患者早期输注血浆冷沉淀可提高患者血浆Fn水平，缩短出血时间，防止凝血异常，加快创伤组织修复。Objective To observe the effects of cryoprotein on the treatment of multiple and surgical traumas. Methods A total of 580U cryoprotein was transfused averagly to 32 multiple and surgical trauma patients, whose ISS90 scores wer 18. 21 ±5. 63.Twelve patients were chosen for determination of their blood routine, bleeding time and fibronectin before and after transfusion respectively. The Fn leval in plasma was tested in six multiple-trauma patients before and after transfusion of 0. 5, 2, 24h respectively.Results The bleeding time was shortened significantly after transfusion(P<0. 05) and the Fn level was higher after transfusion of 0.5h and still higher even 2h later. Conclusion Cryoprotein therapy can shorten bleeding time, avoid abnormality of blood coagulation and promote tissue healing.
