结核病患者血浆和胸水中白介素6水平及其临床意义

摘　　要：目的通过检测结核病患者血浆及胸水中白细胞介素6(IL-6)的浓度变化情况,探讨IL-6与结核菌感染的临床转归、疾病严重程度的关系。方法用ELISA法检测30例健康体检者、30例潜伏感染者、30例活动性肺结核患者、20例结核性脑膜炎患者及20例结核性胸膜炎患者血浆和胸水中的IL-6的浓度。结果肺结核患者血浆中IL-6水平明显高于健康对照组及结核潜伏感染者(P<0.01,P<0.01);结核性胸膜炎患者血浆中IL-6水平明显高于肺结核患者(P=0.0029);结核性胸膜炎患者胸水中的IL-6显著高于相应血浆中IL-6水平(P<0.0001)。结论 IL-6与结核菌感染的临床转归、结核病严重程度相关,是局部病灶炎症反应的重要作用因子。Objective To investigate the concentration of the cytokine IL-6 in plasma and pleural fluid of TB patients,and the relationship between IL-6 and the prognosis of Mycobacterium tuberculosis infection and the severity of active TB disease.Methods The concentration of IL-6 in plasma of 30 healthy donors（HD）,30 individuals with latent tuberculosis infection（LTBI）,30 patients with pulmonary tuberculosis（PTB） and 20 patients with tuberculous meningitis（TBM） was determined by ELISA.IL-6 in cohort plasma and pleural fluid（PF） from 20 patients with tuberculous pleurisy（TP） was also determined.Results The concentration of IL-6 in PTB was significantly higher than that in HD and in those with LTBI（P 0.01,P 0.01） ;IL-6 production was significantly higher in patients with TBM than those with PTB（P = 0.0029） ;IL-6 production was significantly increased in pleural fluid than that in plasma（P 0.0001）.Conclusions It demonstrated that increased IL-6 is associated with the prognosis of Mycobacterium tuberculosis infection and the severity of active TB disease;IL-6 was one of the important inflammation cytokine in the site of infection.
