摘 要:目的探讨高原寒区战时肢体枪弹伤对肝酶谱和肾功能的影响。方法将24只实验猪随机分为高原战时致伤组(GZ)、高原平时致伤组(GP)和平原平时致伤组(PP),分别于伤前、伤后不同时相点采静脉血,测定ALT、γ-GT、LDH和Cr、BUN含量。结果 (1)PP组在伤后早期ALT、γ-GT、LDH均有一个轻度升高,GP组在伤后ALT、γ-GT、LDH变化与PP组基本一致,但变化的程度较重;而GZ组伤前肝酶谱有轻度改变,伤后改变的程度较GP组显著,而且持续的时间较长。(2)PP组动物伤后早期肾功能有轻度改变,2d基本恢复。GP组伤后肾功能改变与PP组基本一致,但改变的程度稍重。GZ组伤后肾功能改变显著重于GP组,而且出现的时间早、恢复时间晚。结论高原高寒地区战时环境肢体枪弹伤后,肝酶谱和肾功能异常出现早、程度重、持续时间长。Objective To investigate the effects of gunshot wounds in swine's hind limbs on liver zymogram and renal function in a wartime environment at high altitude.Methods 24 pigs were randomly divided into GZ group(n=8),GP group (n=8) and PP group (n=8).Pigs in GZ group were sent to high altitude (3670 m above sea level) from plain (351 m above sea level) and lived in a simulated wartime environment for 72 h until their hind limbs were wounded by gunshot.Animals in GP group were also sent to the same high altitude but lived in a normal environment for 2 w and then suffered from gunshot wounds,while those in PP group were fed under normal conditions at 16-22 ℃ in plain (351 m above sea level) and injured thereafter.Venous blood of each pig was collected at 30 min before injury and 2 h,6 h,12 h,1 d,2 d,3 d,5 d,7 d,10 d,14 d after gunshot for the measurement of ALT,γ -GT,LDH,Cr and BUN content.Results (1)ALT,γ -GT and LDH content in PP group decreased slightly early after injury.These three indexes had a similar but severer change in GP group.Liver zymogram in GZ group had a slight change before injury,but it changed more significantly and longer than that in GP group after injury,and the change lasted longer.(2)Renal function in PP group had a slight change early after injury and recovered in 2 d,while GP group had a similar but severer change.The change of renal function in GZ group was significantly severer than that in GP group,and it appeared early and recovered late.Conclusion In a wartime environment at high altitude,the abnormality of swine's liver zymogram and renal function after gunshot wounds in hind limbs occurs early,severely and continuously.
