股骨近端髓内钉PFN治疗老年股骨粗隆间骨折的疗效观察

摘　　要：目的探讨股骨近端髓内钉PFN治疗老年股骨粗隆间骨折的临床疗效，并与动力髋螺钉DHS做术后功能恢复情况比较。方法随访自2008年1月～2009年12月采用股骨近端髓内钉及DHS治疗的43例股骨颈骨折患者，采用Harris评分系统评价两种术式的疗效。结果术前、术后l周和6个月Harris评分：A组分别为35．30±7．91分，82．30±11．21分，86．83±10．52分。B组分别为37．54±8．32分．78．27±12．42分，84．97±9．96分。术后1周A组评分高于B组，且差异有统计学意义（P〈0．05），术前及术后6个月比较差异无统计学意义（P〉0．05）。结论股骨近端髓内钉治疗老年股骨粗隆间骨折的早期临床疗效疗效满意，但其后期疗效有待进一步随访观察。’Objective To compare the treatment of Femoral intertrochantcrle fractures of the aged patients by proximal femoral nail （PFN） and dynamic hip screw （DHS）. Methods fourty - three patients were randomly divided into two groups. Harris scoring of the hip function and the rate of complications were evaluated. Results The scores of PFN group was 35.30±7.91,82. 30±11.21,86. 83 ±10. 52. The scores of DHS group was 37. 54 ±8. 32 ,78. 27 ± 12. 42,84. 97 ±9.96. There was significant difference between the two groups in the one week, and no significant difference in functional assessment in the six months. Conclusion Compared with DHS PFN was a less invasive internal fixation means and superior at early clinical period to treat intertroehanterie fractures.
