2型糖尿病大血管病变患者胰岛素抵抗与中医证候的相关性研究

摘　　要：目的研究T2DM大血管病变患者胰岛素抵抗与中医证候的相关性。方法 60例T2DM大血管病变患者按中医证候分为阴虚燥热、气阴两虚、阴阳两虚3组各20例,另设正常对照组20例,分别用葡萄糖氧化酶法测量每组患者的空腹血糖(FBG)、化学发光法测量胰岛素(FINS),测量身高、体质量,并根据公式计算胰岛素抵抗指数(IRI)、胰岛素作用指数(IAI)及体质量指数(BMI)。比较阴阳两虚型患者IRI与BMI的相关性。结果阴阳两虚组患者IRI、IAI、BMI较阴虚燥热组和气阴两虚组均有显著差异(P<0.01);阴阳两虚组患者的BMI与IRI进行相关性分析,相关系数r=0.696(P<0.01),二者有典型的相关性。结论 IR在T2DM大血管病变的发生中起重要作用,从中医辨证分型来看IR与阴阳两虚型关系最为密切。Objective To research the correlation between the symptom and sign of Traditional Chinese Medicine（ TCM） and insulin resistance（ IR） in type 2 diabetes mellitus with macrovascular complications . Methods 60 patients with type 2 diabetes mellitus macrovascular complications were divided into 3 groups： dryness-heat due to ＂ Yin＂ deficiency group,Deficiency both ＂ Qi＂ and ＂ Yin＂ group and deficiency both ＂ Yin＂ and ＂ Yang＂ group according to Symptom and Sign of TCM,another 20 cases was considered as control group. FBG was assayed by glucose oxidase ,FINS was detected by chemiluminescence; height and weight were measured; IRI,IAI and BMI were calculated,and the correlation between IRI and BMI in deficiency both ＂ Yin＂ and ＂ Yang＂ groups was analyzed. Results The IRI,IAI and BMI in deficiency both ＂ Yin＂ and ＂ Yang＂ groups had remarkable differences compared with Dryness-Heat due to ＂ Yin＂ Deficiency group,Deficiency both ＂ Qi＂ and ＂ Yin＂ group （ P 0. 01） . There was correlation between IRI and BMI （ r = 0. 696） in deficiency both ＂ Yin＂ and ＂ Yang＂ group. Conclusion IR play a key role in the pathogenesis of T2DM macrovascular complications. IR has a tight connection with deficiency both ＂ Yin＂ and ＂ Yang＂ Syndrome of TCM.
