摘 要:目的研究尿毒症大鼠腹膜透析过程中脂多糖对腹膜透析效能以及腹膜结构的影响。方法将采用含腺嘌呤喂养制作尿毒症模型,随后采用自制腹透管行腹膜透析置管术制作腹膜透析模型,使用4.25%腹膜透析液进行腹膜透析。将20只尿毒症腹膜透析模型SD大鼠随机分为两组:脂多糖组(LPS组),分别在33、35、37 d在腹腔内加入LPS 75μg/(d.只);对照组(不加LPS);每组10只。45 d后行腹膜平衡试验,腹膜组织病理学检查,了解腹膜透析效能和结构的变化。结果 (1)LPS组的肌酐的透析液/血液浓度比值(D/P肌酐)和超滤量与对照组比较显著降低(P<0.01)。(2)大鼠腹膜组织病理检查光镜下观察:LPS组可见腹膜间皮细胞肿胀、变形,部分皱缩,连续性不完整,间皮下基质大量积聚,伴有大量炎性细胞的浸润和毛细血管增生。对照组大鼠腹膜间皮细胞完整,细胞间连续性良好,间皮下基质无明显增多。结论脂多糖能够导致腹膜透析时腹膜的受损和腹膜效能的下降。Objective To study the effects of lipopolysaccharide on efficiency of peritoneal dialysis and peritonium ultrastructure in rats with uraemia.Methods Peritoneal dialysis was performed on uraemic rat model,which was created by feeding amidopurine to rats,with 4.25% glucose solution and LPS through self-made dialysis catheter.Twenty male SD model rats were randomly divided into two groups: LPS group and control group.Peritoneal Equilibration Tests(PET) was performed and the change of peritoneal structure was observed.Results 1.D/P creatinine level and ultrafiltration volume in LPS group were significantly lower than those in control group(P〈0.01).2.In LPS group,pathological observation showed swelling and deformation of mesothelial cells,with some lost integrity or shrinkage in peritoneal tissue.Interstitial substance aggregation,inflammatory cell infiltration and capillary hyperplasia were also presented submesothelially.While no specific change of peritoneum was found in control group.Conclusion Lipopolysaccharide can impair peritoneal mesothelium and reduces peritoneal dialysis efficiency.
