人工髋关节置换治疗高龄非稳定性股骨粗隆间骨折的临床疗效

摘　　要：目的分析人工髋关节置换治疗高龄非稳定性股骨粗隆间骨折的临床疗效。方法人工髋关节置换治疗高龄非稳定性股骨粗隆间骨折32例,男性18例,女性14例,年龄62～89岁,平均74岁。骨折类型按Evans-Jenson分类:Ⅲ型10例,Ⅳ型18例,Ⅴ型4例,并对32例患者进行随访,分析其并发症发生情况。结果 32例均顺利完成手术,手术时间16～130min,术中输血平均450ml,并发症发生情况、患肢轻度深静脉血栓4命名,精神障碍2例,肠道菌群失调2例,人工髋关节脱位1例。30例患者获得随访,随访时间3～32个月,平均13个月。所有病例切口均一期愈合,无术中死亡,无感染,无假体松动。结果按Harris评分标准:优11例,良15例,可3例,优良率达86.7%。结论人工髋关节置换治疗高龄非稳定性粗隆间骨折创伤小,并发症少,是可供选择的有效方法之一。Objective To analyze the total artificial hip replacement for the management of unstable intertrochanteric fractures in the elderly.Methods A retrospective analysis of 32 consecutive the elderly cases of the total artificial hip replacement for the management of unstable intertrochanteric fractures was conducted.18 cases were male and 14 cases were female.32 cases had the average age of 74 years （ranged 62~89 years）.Type Ⅲ of the fracture by Evans-Jenson classification was in 10 cases,type Ⅳ in 18 cases,and type Ⅴ in 4 cases.Results Followed-up period ranged from 3 months to 32 months （average 13 months） in 30 cases.None died in operation,relaxation and infection.The results showed 11 cases were excellent,15 cases were good,and 3 cases were fair by Harris＇s criterion.The final satisfactory rate of being excellent and good was 86.7%.Conclusion The total artificial hip replacement for the management of unstable intertrochanteric fractures in the elderly is one of desirable therapeutic method,and it has small operation and complication.
