摘  要:目的探讨多普勒超声心动图在评价缺氧新生儿左心室功能障碍中的作用。方法用多普勒超声心动图对轻、中、重度低氧血症组新生儿(52例)及正常对照组新生儿(40例)于生后1、3、7d分别测定反映左心室收缩功能的射血分数(EF)及反映舒张功能的二尖瓣口E/A值,并比较各组以上指标的差异。结果轻度低氧血症组生后1、3、7d左心室EF、E/A值与对照组相比差异无统计学意义;中度低氧血症组生后1、3d左心室EF、E/A值与对照组比较差异有统计学意义(P<0.05),生后7d比较差异无统计学意义;重度低氧血症组生后1、3、7d左心室EF、E/A值与对照组相比较差异均有统计学意义(P<0.05)。结论多普勒超声心动图可发现中、重度低氧血症患儿左心室收缩及舒张功能障碍,对临床诊治有重要的指导作用。Objective To assess the clinical diagnosis value by doppler echocardiography in left ventricular dysfunction in neonatal hypoxemia.Methods Doppler echocardiography was performed to measure the systolic function of ejection fraction(EF)and diastolic function of the E/A in mitral valve of left ventricular,in mild,moderate and severe neonatal hypoxemia newborns group(52cases)and normal control infants(40cases)1,3,7dafter birth,compare the differences in those indicators in each group.Results There were no significant differences in EF and diastolic function of E/A values on 1,3,7day after birth between mild hypoxemia group and control group;Similarly.There were no significant differences in EF and diastolic function of E/A values on 7day after birth between moderate hypoxemia group and control group.But on 1,3day after birth there were significant differences in EF and diastolic function of E/A values in above both group(P0.05);There were significant differences in EF and diastolic function of E/A values on 1,3,7day after birth between severe hypoxemia group and control group.Conclusion Doppler echocardiography can discover systolic and diastolic dysfunction of left ventricular in newborns with severe hypoxemia,which may play important guiding role in clinical diagnosis and treatment in newborns with severe hypoxemia.
