刚性固定与动力固定治疗腰椎管狭窄症效果研究

摘　　要：目的:对比刚性固定和动力器械在全腰椎和节段腰椎脊椎外形的短期效果,客观评价结果和有关的并发症。方法:研究对比了三组例数相同的患者,每组为45例成年病,观察手术以及术后疗效评价。结果:经过对平均随访(47±14)个月后所有患者的评价,腰椎和全脊椎前凸纠正程度与器械固定的节段水平数目无关;在所有病例中直至最后一评价均没有出现相邻节段的脊椎退变。结论:这个对照研究表明:三组器械在短节段的应用在治疗退变椎管狭窄中,对于维持术前腰骶段全段及局部的脊椎弧度、随后自我评价及疼痛评分中改善效果基本相同。单独的不伴有影像学上可见的假关节及多抵抗力的内固定物失效在动力器械组中发生率比较低。Objective Directed to Rigid fixation and dynamic stabilization instruments in the whole lumbar spine and lumbar spine segmental form of short-term effects,objective evaluation of results and related complications.Methods Study compared three groups of patients the same number of cases in each group of 45 cases of adult disease,observed effect of surgery and post-operative evaluation of.Results After a mean follow-up （47±14）months after the evaluation of all patients,lumbar spine lordosis and the whole apparatus to correct degree and the level of a fixed number of segments has nothing to do;in all cases until the last evaluation did not appear adjacent segments of the spine Degeneration.Conclusion The control study showed that：three groups of instruments in a short segment of the application in the treatment of degenerative spinal stenosis,surgery Qianyao sacral for the maintenance of the whole paragraph and local curvature of the spine,followed by self-evaluation and improvement of pain score in the effect of basically the same.Accompanied by a separate non-visible imaging of false joints and more resistant to internal fixation failure in the dynamic stabilization group,the incidence is relatively low.
