摘 要:目的观察吸湿冷凝加湿器联合密闭式吸痰在预防呼吸机相关性肺炎(VAP)中的意义。方法将入住ICU行机械通气超过48h的96例患者,按随机数字表法分为观察组(49例)和对照组(47例)。观察组采用吸湿冷凝加湿器联合密闭式吸痰,对照组采用加温加湿器联合开放性吸痰。比较两组VAP发生率、VAP发病时间及住院时间,机械通气不同时间通气参数及血气指标。结果观察组VAP发生率显著低于对照组,出现VAP的时间显著长于对照组,住院时间显著短于对照组(P<0.05,P<0.01);不同机械通气时间两组通气的各项参数及血气分析指标比较,差异无统计学意义(均P>0.05)。结论吸湿冷凝加湿器联合密闭式吸痰,能够延缓并降低VAP的发生,缩短患者住院时间。Objective To explore the effect of heat and moisture exchanger combined with closed endotracheal suctioning on prevention of ventilator-associated pneumonia (VAP).Methods A total of 96 patients who had mechanical ventilation for at least 48 h were randomly divided into two groups. The observation group (49 cases) were given heat/moisture exchanger combined with closed endotracheal suctioning, while the control group(47 cases) applied heated humidifier and open endotracheal suctioning.Results The occurrence rate of VAP in the observation group was significantly lower than that in the control group, the occurrence time of VAP was significantly postponed, and the length of hospital stay in the observation group was significantly shortened than those in the control group(P〈0.05,P〈0.01).The values of ventilation parameters and blood gas analysis showed no significant differences between the two groups at different time points(P〉0.05 for all).Conclusion Using heat/moisture exchanger combined with closed endotracheal suctioning can decrease the incidence of ventilator-associated pneumonia, and shorten the length of hospital stay.
