常温体外循环下直视闭合术治疗粗大型动脉导管未闭98例分析

摘　　要：目的：探讨对粗大型动脉导管未闭患者行常温体外循环下动脉导管未闭直视闭合术的临床效果。方法：自2003年1月～2010年6月，对98例粗大动脉导管未闭采用常温体外循环下行动脉导管未闭直视闭合术。体外循环开始后，切开肺动脉，用流出道探子堵住动脉导管开口后再行直视修补。结果：全组均治愈，15例术后出现高血压，无一例出血、喉返神经损伤、气栓、灌注肺等并发症。结论：常温体外循环下直视闭合术治疗粗大型动脉导管未闭是一种安全、可靠的手术方法。Objective： To investigate the clinical effects of direct closure operation treatment large patent duetus arteriosus（PDA） under normothermic cardiopulmonary bypass （CPB）. Methods： 98 patients with large PDA were directly repaired under normothermic cardiopulmonary bypass（CPB） between January 2003 and June 2010. Incision pulmonary artery after cardiopulmonary bypass, the direct repairment was performed through pulmonary artery approach which was blocked with a dilator. Results： fifteen patients appeared hypertension after the direct closure operation, No one surgical death occurred, and no one had complications as hemorrhage recurrent, laryngeal nerve injury, aeremia, perfusion lung, etc... they were discharged with good recovery. Conclusion： The direct closure operation under normothermic CPB treatment large PDA is a more reliability and effective method.
