摘  要:目的探讨肿瘤坏死因子相关凋亡诱导配体(tumor necrotic factor-related apoptosis-inducing ligand,TRAIL)在颈椎间盘退变中的作用。方法采用原位DNA片段末端标记(TUNEL)法和免疫组织化学方法,对比观察30例颈椎病退变颈椎间盘组织细胞及30例创伤对照颈椎间盘组织细胞凋亡状态及TRAIL的表达。结果 TUNEL染色观察示颈椎病组的髓核内的平均凋亡细胞率为(10.93±1.87)%,高于对照组的(6.43±1.56)%(P<0.01)。SP免疫组织化学染色示颈椎病组髓核内TRAIL染色阳性细胞率为(49.13±15.66)%,高于对照组的(20.26±2.81)%,差异有统计学意义(P<0.01)。将对照组和颈椎病组全部60例颈椎间盘标本合并后进行Pearson相关分析,颈椎间盘髓核内TRAIL染色阳性细胞率与TUNEL阳性细胞率之间呈正相关,相关系数r=0.921(P<0.01)。结论颈椎病椎间盘组织细胞凋亡数与TRAIL阳性细胞数均明显高于创伤对照组,且凋亡与TRAIL的表达呈正相关。Objective To explore the expression of tumor necrotic factor-related apoptosis-inducing ligand(TRAIL) in degenerative cervical disc tissue.Methods The apoptotic status and the expression of TRAIL were detected in 30 cases of degenerative cervical intervertebal disc tissues and 30 of traumatic normal cervical intervertebal disc tissues with TdT-mediated dUTP-biotin nick end labeling(TUNEL) and immunohistochemistry methods.Results The positive rate of TUNEL staining was higher in patient group(10.93±1.87) % than that in control group(6.43±1.56) %(P0.01).The positive expression rate of TRAIL of nucleus pulposus in patient group(49.13±15.66)% was higher than that in control group(20.26±2.81) %(P0.01).When the total 60 specimens in the two groups were analyzed,the TRAIL positive rate of nucleus pulposus showed significant positive correlation with the TUNEL positive incidence of nucleus pulposus(r=0.921,P0.01).Conclusion The positive rate of TUNEL staining and TRAIL of nucleus pulposus in patient group were higher than that in control group.The TRAIL positive incidence of nucleus pulposus showed significant positive correlation with the TUNEL positive incidence of nucleus pulposus.
