超声斑点追踪显像评价高血压左心室心肌肥厚患者左心室舒张功能

摘　　要：目的应用超声斑点追踪显像(STI)探讨高血压伴左心室心肌肥厚(LVH)对左心室舒张功能的影响。方法 45例按照LVH程度分为无、轻度及中-重度LVH组的高血压患者与20例正常人采集左心室短轴基底及心尖平面二维图像,运用EchoPAC工作站脱机测量等容舒张末扭转角度、解旋峰速、标化达解旋峰速时间、5%、15%及30%舒张期的解旋程度及等容舒张期解旋率。结果高血压轻度及中-重度LVH组与正常组相比,解旋率与舒张早期的解旋程度均显著减低(P<0.01),标化达解旋峰速时间显著延迟(P<0.01),解旋峰速及等容舒张末扭转角度增大(P<0.05)。高血压各组解旋率与左心室质量指数间存在显著负相关关系(r=-0.454,P=0.001)。结论 STI可以无创性定量评价高血压LVH所导致的舒张早期左心室解旋减小及延迟,可作为评价左心室舒张功能的新工具。Objective To assess the left ventricular （LV） diastolic function in hypertensive patients with left ventricular hypertrophy （LVH）using speckle tracking imaging （STI）echocardiography.Methods A total of 45 hypertension patients divided into 3 groups according to the degree of LVH （no LVH group,mild LVH group and moderate-severe LVH group）were included and 20 healthy subjects were as control group.Two-dimensional images of the LV short axis views at basal and apical levels were recorded and analyzed by 2Dspeckle tracking soft in EchoPAC workstation,we acquired the twist at the end of isovolumic relaxation（TwistMVO）,peak untwisting velocity （PUV）,normalized time-to-peak untwisting velocity （nTPTV）,5%,15%,30% diastolic untwist and untwist rate.Results Compared with the control group,untwisting rate and untwisting early in diastole were significantly reduced in mild LVH group and moderate-severe LVH group（P 〈 0.01）.The duration of TPUV was significantly prolonged（P 〈 0.01）and PUV and TwistMVO increased（P 〈 0.05）.The untwist rate was significantly negatively correlated with LV mass index in all hypertensive groups（r =-0.454,P =0.001）.Conclusion Speckle tracking imaging echocardiography can be used for the non-invasive and quantitative assessment of the reduced and delayed diastolic untwisting during isovolumic relaxation period in hypertensive patients with LVH,and STI is a new tool to evaluate LV relaxation.
