成人变应性鼻炎患者症状及生活质量评估

摘　　要：目的:评估成人变应性鼻炎(allergic rhinitis,AR)患者的症状和生活质量及两者的相关性,同时评估皮肤点刺试验结果与症状、生活质量的相关性。方法:用症状视觉模拟评分量表(visual analog scale,VAS)评价患者的症状严重程度,用鼻结膜炎相关生活质量问卷(Rhinoconjunctivitis Quality of Life Questionnaire,RQLQ)评价患者的生活质量,记录皮肤点刺试验结果,并进行统计学分析。结果:(1)VAS各项症状间比较有显著性差异(F=264.89,P<0.01),喷嚏是患者的首要症状(64.47±26.14)。(2)RQLQ 7方面间及各项目间比较均有显著性差异(F=29.15,P<0.01;F=34.65,P<0.01),生活质量最受影响的方面是鼻部症状(2.72±1.68),评分最高的项目是喷嚏(3.27±1.49)。(3)两量表有显著相关,其中,难以入睡、夜间醒来次数、睡眠质量不高与鼻塞VAS评分呈正相关(r=0.310,P=0.000;r=0.323,P=0.000;r=0.329,P=0.000),工作效率下降、注意力难以集中与难以入睡、夜间醒来次数、睡眠质量不高呈正相关,无耐心或烦躁与鼻塞、喷嚏VAS评分呈正相关(r=0.228,P=0.001;r=0.145,P=0.036),易怒与鼻塞VAS评分呈正相关(r=0.180,P=0.009),因症状而感到尴尬与喷嚏VAS评分呈正相关(r=0.150,P=0。030)。(4)皮肤点刺试验结果与两量表均无显著相关。结论:成人AR患者鼻部症状的严重程度与其生活质量显著相关,其中鼻塞、喷嚏可能是影响患者生活质量的主要原因;变应原级别与症状严重程度及生活质量无显著相关。Objective：To evaluate symptoms and quality of life in adult with allergic rhinitis,and to evaluate the correlation among the skin prick tests results ,symptoms and quality of life.Methods：Visual analog scale（VAS） was used to assess the symptoms severity,rhinoconju nctivitis quality of life questionnaire （RQLQ） was used to assess quality of life,the skin prick tests results were recorded,and statistics analysis was carried out among them.Resnits：（1）There were significant differences among the items of VAS（F=264.89,P〈0.01）, sneezing was the major symptoms for adult patients（64.47±26.14）.（2）There were significant differences among the severn aspects of RQLQ（F=29.15,P〈0：01） ,nasal symptoms item was the most affected items of quality of life（2.72±1.68）,and there were significant differences among all the items of RQLQ（F=34.65,P〈0.01）,the score of sneezing items was the highest（3.27±1.49）.（3）VAS was significantly correlated with RQLQ,moreover, the scores of unable to get to sleep, wake up during the night and lack of a good night＇s sleep were significantly correlated with nasal blockage VAS score（r=0.310,P=0.000;t=0.323,P=0.000;r=0.329,P=0.000）,the scores of get less accomplished and can＇t concentrate were significantly correlated with the scores of unable to get to sleep,wake up during the night and lack of a good night＇s sleep,the score of impatient was significantly correlated with nasal blockage and sneezing VAS scores（r=0.228,P=0.001 ;r=0.145,P=0.036）,the score of irritable was significantly correlated with nasal blockage VAS score（r=0.180,P=0.009）,the score of embarrassed by symptoms was significantly correlated with sneezing VAS score（r=0.150,P=0.030）.（4）The results of the skin prick tests was irrelated with VAS and RQLQ.Conelusion：Nasal symptoms severity may significantly correlated with the quality of life for adult AR patients,moreover, nasal blockage and sneezing may be the main factor affecting the quality of life.The a
