应用ROC曲线评价血清胱抑素C对糖尿病早期肾功能损害的诊断价值

摘　　要：目的:用ROC曲线评价血清胱抑素C(CysC)水平在诊断糖尿病肾病早期肾功能损害的临床应用价值。方法:采用颗粒增强散射比浊法测定100例糖尿病肾病早期患者的血清CysC水平,同时测定血尿素氮(Bun)和血肌酐(Scr)水平,通过分析受试者特征工作曲线(ROC曲线)评价它们对评价糖尿病肾病早期肾功能损害的价值。结果:(1)糖尿病肾病早期组中CysC高于对照组(P<0.01);(2)血清Bun、Scr、CysC的ROC曲线下面积分别为0.716、0.781、0.954,差异有统计学意义(P<0.01)。结论:血清CysC是反映糖尿病肾病患者早期肾损害的良好指标,而且优于血Bun、Scr。Objective：To evaluate the diagnostic value of the serum cystatin C on the early stage of diabetic nephropathy by using the ROC curve.Methods：Serum Cys C levels of 100 patients with early stage of diabetic nephropathy were detected by particle enhanced immunonephelometric assay ,Bun and Scr were measured by biochemistry analyzer.we evaluate the diagnostic value of them on in the early stage of diabetic nephropathy by analyzing the ROC curve.Results：①The concentration of serum Cys C in the early stage of diabetic nephropathy was significantly higher than that in the normal control group （ P 0.01）.②The area under the ROC curve of Bun、Scr and Cys C in serum were 0.716,0.781 and 0.954.There was a significant difference among them （ P 0.01）.Conclusion：Cys C might be a good serological indicator for evaluating early stage damage in kidney of diabetic nephropathy and it should be better than Bun and Scr.
