六指六穴点压及膝关节旋转屈伸手法治疗膝关节骨性关节炎的实验研究

摘　　要：目的:观察六指六穴点压及膝关节旋转屈伸手法对预防实验性家兔膝关节骨性关节炎进展的影响,探讨其防治膝骨性关节炎的作用机制。方法:选用健康家兔29只,随机分为空白对照组(A组)9只、模型组20只。然后将模型组动物随机分为模型对照组(B组)8只、手法治疗组(C组)8只。正常对照组和模型对照组正常饲养,不做处理。手法治疗组(C组)采取手法治疗隔日一次,连续治疗21d。结果:①各造模家兔体质量在治疗前后均明显低于空白对照组,模型对照组体质量呈下降趋势,手法治疗组家兔体质量较治疗前增加明显。②B、C组家兔在治疗前均出现较重的跛行,C组在治疗后跛行的数量少于B组,且在程度上轻于B组。③关节冲洗液中炎性因子和基质金属蛋白酶-3含量检测:治疗前B、C组动物关节冲洗液中IL-1及MMP-3含量与A组相比有显著性差异(P<0.05),B、C组间比较无显著性差异(P>0.05)。治疗后,C组关节冲洗液中IL-1及MMP-3含量与B组比较有显著性差异(P<0.05),与A组比较无显著性差异(P>0.05),与治疗前比较有显著性差异(P<0.05)。结论:六指六穴点压及膝关节旋转屈伸手法通过活血散瘀、舒筋通络、宣通散结、激发经气、平衡阴阳的作用机理降低关节冲洗液中炎性因子及基质金属蛋白酶的水平,进而减少对关节软骨的持续破坏,有效防止关节软骨的退变。Objective ： To observe the influence of acupuncture points and six knee flexion technique for the prevention of rotating experimental rabbit knee osteoarthritis progression, discusses the mechanism of osteoarthritis of the knee. Methods ： Twenty - nine health rabbits were randomly divided into the blank group （group A） 9, model group20. Then the model group animals were randomly divided into model control group （group B） only 8, the treatment group （8） group C. The normal control group and the control model, do norreally bred. the treatment group （C） take manipulation treatment tertian, continuous treatment for three weeks. Results ： （ 1 ） The rabbit weight in the treatment group were significantly lower than blank, model control weight is decreasing , treatment operation group were significantly increased weight than before. （2） B and C in the treatment group all appear the heavy limp, the treatment group C after limping less in group B, and lighter than group B degree. （ 3 ） The joint rinses proinflammatory cytokines and matrix metalloproteinases - 3 content detection ： animal of treatment group B and C of joint rinses IL - 1 and 3 - MMP compared with A has significant difference （ P 〈 0. 05 ） comparision between groups, B and C has non - significance difference （ P 〉 0.05 ）. After treatment, the group C joint rinses in 1 - IL MMP and 3 - content compared with group B was significant difference （ P 〈 0.05 ）, with A group has non - significance difference （ P 〉 0.05 ） with before treatment, have significant difference （ P 〈 0.05 ）. Conclusion ： Six points and six point by rotating the knee flexion blood stasis, scattered gimmick bacteric, XuanTong dissipating, qi, balancing Yin and Yang of the mechanism of reducing joint rinses proinflammatory cytokines and matrix metalloproteinases, and reduce the level of articular cartilage, effectively prevent the destruction of the continuous of articular cartilage degeneration.
