摘  要:目的:探讨数字化技术在腓肠神经筋膜皮瓣移植中的的可行性及其临床意义。方法:明胶一氧化铅混悬液灌注的新鲜成人尸体标本2具,16排多层螺旋CT进行全身连续扫描,观测腓肠神经营养血管分布及彼此间的吻合情况,应用Amira 3.1软件对腓肠神经筋膜皮瓣结构进行三维重建并立体显示。5例足跟部皮肤缺损、骨外露病人采用腓肠神经营养皮瓣移植进行创面修复。结果:腓肠神经营养皮瓣的解剖学结构特点可通过三维重建技术准确反映。临床应用5例病人经6~24mo随访,所有皮瓣成活良好。结论:数字化技术可以为腓肠神经营养皮瓣移植的临床应用提供直观的、数字化解剖依据。Objective:To discuss the digitalization technique used in the visualization of sural neurofasciomyocutaneous flap and their blood supply. Methods:2 specimens of adult fresh cadaver, were subjected to radiographic CT scanning before and after perfusion with lead oxide - gelatine mixture. Amira 3.1 software was used to transform the 2D images in DICOM format into 3D models of the entire region. The structures of sural neurovascular flap were observed and the digitalized visible models of sural neurovascular flap was established through three - dimensional computerized reconstruction. Five cases of soft - tissue defects of the foot calcaneal region, involving the exposure bones of the heel, were treated by the sural neurofasciomyocutaneous flap. Results:The three - dimensional reconstruction of visible models established from these datasets perfectly displayed the characteristics of sural neurovascular anatomy. All 5 cases were followed - up about 6 to 24 months. All flaps survived well. Conclusion:The digitized images can offer unique insights into the neurofasciomyocutaneous flap anatomy.
