晚期早产儿周岁生长发育状况调查

摘　　要：【目的】了解晚期早产儿1周岁时体格生长和智能发育情况。【方法】选取本院满周岁207名晚期早产儿作为研究对象,分成正常出生体重早产儿组和低出生体重早产儿组:1)进行身长、体重、头胸围的测量,并与上海市平均值进行比较;2)对晚期早产儿进行丹佛发育筛查(Denver Developmental Screening Test,DDST),并与正常足月儿进行比较。【结果】1)除正常体重早产男婴组体重、胸围,女婴体重、头围与上海市平均值比较差异无统计学意义(P>0.05),其余体格发育指标与上海市平均值差异有统计学意义。正常出生体重组和低出生体重组晚期早产儿周岁时男婴的体重、女婴的体重、身长、胸围比较差异有统计学意义,其余指标差异无统计学意义。2)晚期早产儿DDST测试结果与正常足月儿差异有统计学意义(P<0.05)。【结论】与正常足月儿相比,晚期早产儿1周岁时的体格生长和智能发育均受到一定影响,因此有必要加强对晚期早产儿的随访管理以期达到正常儿童的发育水平。[Objective] To obtain the growth and development of the late preterm children in their 12-month old. [Methods] 207 late preterm children were invited to our research who were followed up in our clinic, and divided into two groups, normal birth weight group and low birth weight group, according to their birth weight. Data was collected when they were 12-rnonth old. Evaluations were performed on the height, weight, head and chest circumference, as well as Denver Developmental Screening Test （DDST）. [Results] 1） Comparing with peers in Shanghai, there was a significant difference in the detected items, except items of weight, chest circumference in preterm boys with normal birth weight, and weight, head circumference in preterm girls with normal birth weight（P〉0.05）. Comparing with the preterm peers born with normal birth weight, significant differences were found in weight in boys, and height, weight, chest circumference in girls with low birth weight, when they were one-year old. 2）There was a significant difference in DDST result between late preterm children and peers （P%0.05）. [Conclusions] The results indicate that the growth and development of the late preterm children are somewhat affected, comparing with peers born in full-term. Strengthen the follow-up management of late preterm children to achieve the maximum development with the peers born in full-term is required.
