组织多普勒与脉冲多普勒Tei指数对肝硬化患者左心功能改变的对比研究

摘　　要：目的采用脉冲波多普勒法(PW)与组织速度成像(TVI)测算左心室Tei指数评价肝硬化患者心功能。方法肝硬化患者150例,慢性肝炎患者180例,分别应用TVI和PW技术测算左心室Tei指数。结果肝硬化患者左心室的Tei指数大于慢性肝炎患者(P<0.01);肝硬化Child A、B级患者及慢性肝炎患者TVI、PW两种方法所测Tei指数比较差异无统计学意义(P>0.05);Child C级组与A、B级组及慢性肝炎组左心室的Tei指数比较P<0.01;TVI检测显示,Child B级组与慢性肝炎组的Tei指数比较,差异有统计学意义(P<0.05),PW法检测两组间,比较P>0.05;肝硬化患者按腹水量分组,各组间Tei指数比较差异无统计学意义(P>0.05)。结论肝硬化患者存在左心功能异常,与肝功能损害严重程度相一致,与腹水量无明显相关性;TVI法测算的Tei指数较PW法简便、敏感。Objective To evaluate the left ventricular function of patients with liver cirrhosis by tissue velocity imaging （TVI） and pulsed wave Doppler flow imaging （PW）with Tel index. Methods A total of 150 patients with liver cirrhosis and 180 patients with chronic hepatitis （control group） were enrolled in this study. Their LV Tei index was measured by TVI and PW. Results LV Tel index in liver cirrhosis group was higher than that in control group （ P 〈 0.01 ） , there was no significant difference of Tei index among the Child A and Child B liver cirrhosis patients and chronic hepatitis patients by TVI and PW（ P 〉 0.05 ）. There was significant difference of Tei index among Child C liver cirrhosis patients, Child A and Child B liver cirrhosis patients and chronic hepatitis patients （ P 〈 0.01 ）. TVI showed there was significant difference of Tel index between child B liver cirrhosis patients and chronic hepatitis patients （P 〈0.05）, while there was no significant difference by PW（P 〉0.05 ）. When the patients with liver cirrhosis were classified according to abdominal dropsy, there was no significant difference of Tei index among the groups （ P 〉 0.05 ）. Conclusion The LV functions of patients with liver cirrhosis is abnormal, there is relations between the cardiac function of liver cirrhosis patients and the degrees of hepatic dysfunction, and no obvious correlation with abdominal dropsy. TVI can measure Tei index conveniently and sensitively.
