彩色多普勒超声对肾小结石的诊断价值

摘　　要：目的评价彩色多普勒超声对肾小结石的诊断价值。方法对100例肾小结石(直径2～6mm)患者,135枚小结石,行二维超声和彩色多普勒超声检查。彩色多普勒观察结石后方快闪伪像确定结石的存在,对比分析二维超声和彩色多普勒超声对肾小结石的检出率。通过增加回声差异强化值,观察结石边界情况。结果 135枚小结石,二维超声检出92枚(68.2%,92/135),彩色多普勒通过观察快闪伪像检出128枚(94.8%,128/135),两者比较差异有统计学意义(P<0.05)。结石直径2～3mm小结石74枚,二维超声检出39枚(52.3%,39/74),彩色多普勒超声检出72枚(97.3%,72/74),两者比较差异有统计学意义(P<0.05)。135枚小结石通过增加回声差异强化值(△=4～5)观察,边界均清晰显示。结论①彩色多普勒超声对肾小结石的检出和诊断有重要价值。②回声差异强化可清晰显示肾小结石边界情况,有助于准确测量结石大小。Objective To evaluate the diagnostic value of color Doppler ultrasound in kidney small stone. Methods One hundred and thirty - five small stones in 100 patients with kidney small stones were enrolled in this study （ diamter 2 6 mm）, they were examined by two - dimensional ultrasound and color Doppler ultrasound. The stone existence was determined by the twinkle artifact behind the stone with color Doppler ultrasound, then the detection rate of kidney small stone was comparatively analyzed by two - dimensional ultrasound and color Doppler ultrasound. The situation of stone boundary was observed by increasing echo difference strengthening value. Results In 135 small stones, there were 92 lesions found by two - dimensional ultrasound （68. 2% ,92/135 ）, and 128 lesions found by color Doppler ultrasound with twinkle artifact observation （94.8%, 128/135 ）, there was significant difference between two groups （P 〈 0.05）. In 74 small stones with diameters of 2 3 ram, there were 39 lesions found by two -dimensional ultrasound （52.3% ,39/74）, and 72 lesions found by color Doppler ultrasound （97.3% ,72/74）, there was significant difference between two groups （P 〈0.05 ）. The boundary was displayed clearly in 135 small stones （ △ = 4 - 5 ） with the increased echo difference strengthening value. Conclusion Color Doppler uhrasound has important value in detection and diagnosis of kidney small stone. Echo difference strengthening carl clearly display the boundary situation of kidney small stone, it is helpful in accurate measurement of stone size.
