摘 要:目的观察恐惧应激对支气管哮喘(哮喘)大鼠呼吸道峰压值、血清IL-4、糖皮质激素(GC)水平及肺组织病理改变的影响,探讨恐惧应激因素对大鼠哮喘的影响。方法选取健康雄性SD大鼠32只,随机分为对照组、哮喘组、恐惧应激组和恐惧应激哮喘组。采用卵白蛋白(OVA)腹腔注射致敏加雾化吸入激发方法复制大鼠哮喘模型,以浸水刺激模拟恐惧应激。采用四导联生理记录仪测定大鼠呼吸道峰压值;采用ELISA法检测大鼠血清IL-4、GC水平;常规病理切片观察肺部炎症情况。结果经过OVA致敏激发的哮喘组大鼠呼吸道峰压值、血清IL-4水平较对照组均显著增高(Pa<0.01),肺组织病理形态学可观察到其肺内各级支气管及小血管壁出现少至中等量炎性细胞浸润;恐惧应激哮喘组大鼠血清GC、IL-4水平及呼吸道峰压值均较哮喘组显著增高(Pa<0.01),病理形态学可观察到其肺内各级支气管及小血管壁出现大量炎性细胞浸润,部分支气管周围肺组织出现实变。结论恐惧应激能够加重哮喘大鼠肺组织炎性变化,导致呼吸道峰压值进一步增高,其机制可能是通过增加内源性GC释放,促进Th2细胞活化,从而增加IL-4的合成实现的。Objective To investigate the effects of fear stress on airway peak pressure,the levels of interleukin-4(IL-4) and glucocorticoid(GC),and the histopathological changes of lung tissues in rats with asthma,as well as explore the effects of fear stress on asthma in rats.Methods Thirty-two healthy male grown-up SD rats were randomly divided into control group,asthma group,fear stressed group and fear stressed asthma group.Rats sensitized with ovalbumin(OVA) via intraperitoneal injection were then exposed to OVA.Soak the rats in water to generate a feeling of fear stress.Airway peak pressure was measured with 4-lead graph system,the levels of IL-4 and GC were detected by ELISA,and the histopathological changes of lung tissues were observed by HE staining.Results After sensitization with OVA and expose to it,airway peak pressure and the level of IL-4 of rats in asthma group increased compared with control group(Pa〈0.01),histopathological observation revealed that bronchial tubes at all levels and pulmonary small vascular walls of the rats in asthma group were infiltrated by moderate inflammatory cells;the levels of GC,IL-4,and airway peak pressure of the rats in fear stressed asthma group were higher than those in asthma group(Pa〈0.01).Such histopathological changes in fear stressed asthma group could be observed: more inflammatory cells were infiltrating bronchial tubes at different levels and pulmonary small vascular walls,and even lung consolidation was discovered around some bronchial tubes.Conclusions Fear stress aggravates the allergic airway responses and results in a higher level of airway peak pressure,which is probably produced by increasing the release of endogenous GC and suppressing the activation of Th2 cells,and thus increasing the synthesis of IL-4.
