摘　　要：目的总结保留脾脏的胰体尾切除术经验,探讨其可行性与安全性。方法回顾性分析中国医科大学附属第一医院1993年4月至2009年7月行保留脾脏的胰体尾切除术22例病人的临床资料。结果 22例病人中,无功能胰岛细胞瘤8例、囊腺瘤8例、胰岛素瘤5例、胰腺假性囊肿1例,均成功完成保留脾脏的胰体尾切除术,其中保留脾脏血管18例,切断脾脏血管4例。无围手术期死亡。术后发生胰瘘5例,经非手术治疗2～3周痊愈。结论对于胰体尾部良性病变者,不适合行摘除术时应首先选择保留脾脏的胰体尾切除术,安全可行。Objective To evaluate the outcome of spleen-preserving distal pancreatectomy and explore its feasibility. Methods The clinical data of 22 cases of spleen-preserving distal pancreatectomy from April 1993 to July 2009 in the First Affiliated Hospital of China Medical University were analyzed retrospectively. Results Among them, 8 cases of non-functioning islet cell tumor, 8 cases of cystadenoma, 5 cases of insulinoma and 1 case of pseudocyst were included. There were no postoperative deaths, pancreatic fistula occurred in 5 patients, and was cured within 2～3 weeks by non-operative treatment. Conclusion Spleen-preserving distal pancreastectomy is the preferred procedure for certain benign lesions of pancreatic body or tail that are not candidates for enucleation. This procedure has good result at short-term after the operation.
