左心室收缩功能正常的不稳定型心绞痛患者BNP水平与冠状动脉病变受累范围及经皮冠状动脉介入治疗的关系

摘　　要：目的分析左心室收缩功能正常的不稳定型心绞痛患者脑钠素(BNP)水平与冠状动脉病变受累范围及经皮冠状动脉介入治疗的关系。方法将攀枝花市十九冶医院2009年10月至2010年8月经皮冠状动脉介入治疗的左心室收缩功能正常的不稳定型心绞痛患者72例,根据冠状动脉造影结果分为两组,单支病变组(n=32)和多支病变组(n=40)。比较两组患者的BNP水平。结果经皮冠状动脉介入治疗前单支病变组的BNP水平明显低于多支病变组(P<0.05),经皮冠状动脉介入治疗后3d两组BNP水平均明显升高(均P<0.05),而治疗后30d两组BNP水平均明显低于术前(均P<0.05)。结论左心室收缩功能正常的不稳定型心绞痛患者BNP水平增高与冠状动脉病变受累范围有关,且受经皮冠状动脉介入治疗的影响而波动,可以作为该类患者一项重要的观察指标。Objective To analyze the relationship between the BNP levels of normal left ventricular function in patients with unstable angina and the range of coronary artery involvement and the scope of percutaneous coronary intervention. Methods From October 2009 to August 2010 in Hospital of China 19th Metallurgical Construction Co., Ltd., 72 cases of normal left ventricular function in patients with unstable angina treated by percutaneous coronary intervention were divided into two groups according to coronary angiography, single-vessel disease group （n=32） and multi-vessel disease group （n=40）. The BNP levels of two groups were compared. Results Before percutaneous coronary intervention, the BNP levels of single-vessel disease group were significantly lower than the multi-vessel disease group （P0.05）, 3 days after percutaneous coronary intervention, the BNP levels of two groups were significantly higher （all P0.05） , and 30 days after treatment, the BNP levels of two groups were significantly lower than before surgery （all P0.05）. Conclusion The increase of BNP levels is closely related with the range of coronary artery involvement of normal left ventricular function in patients with unstable angina, and fluctuates by the impact of percutaneous coronary intervention, can be used as an important observation indexes of these patients.
