摘  要:目的:探讨彩色血流信号平均密度(MCVD)与前列腺特异性抗原(PSA)在无创诊断前列腺癌中的临床价值。方法:分析26例前列腺癌及对照组的声像图表现与PSA的测值,进行二维、彩色能量多谱勒检查,观察声像图特点,测量血流丰富区域收缩期峰值血流速度(PS)、舒张末期血流速度(ED)、阻力指数(R I)及搏动指数(P I),计算肿瘤组织及非肿瘤组织单位面积内彩色血流数,即肿瘤及非肿瘤的彩色血流信号平均密度(M CVD),并进行统计学分析。结果:26例前列腺癌中24例病灶在外腺,22例呈低回声结节,与对照组比较血流参数有显著差异(P<0.05),PSA及M CVD有显著差异(P<0.05),随分化程度降低,PSA及M CVD均增加,M CVD与PSA呈正相关(r=0.6826,P<0.01)。结论:M CVD能反映前列腺癌血管生成状况,经直肠超声为无创诊断前列腺癌最佳方法。Objective:To explore the clinical value of mean color vessel density(MCVD) and prostate specific antigen(PSA) in prostate cancer(PCA) by non-invasive diagnostic method.Methods:The echography features and the PSA level were analyzed in 26 cases of PCA and controls.They were examined by transcrectal ultrasound(TRUS) with two-dimensional echography(2DE) and Color Power Doppler(CPD).The peak systolic velocity(PSV),end-diastolic velocity(EDV),resistive index(RI)and pulsative index(PI) were measured in rich blood flow signal region.Mean color vessel density(MCVD) was calculated. The data were analyzed statistically.Results:In the 26 cases of PCA,the lesions were located in prostatic outer gland in 24 cases and 22 cases showed a hypoechoic lesion,with significant differences in the parameters of hemodynamics between the two groups(P0.05),and there was a significant difference between MCVD and PSA(P0.05),and MCVD and PSA showed a positive rank correlation(r=0.501,P0.01). Conclusion:MCVD can reflect the cancer neoangiogenesis in PCA,and TRUS is the best non-invasive diagnostic method in prostate cancer.
