高敏C反应蛋白在早产儿细菌感染性疾病诊断中的意义

摘　　要：[目的]探讨高敏C反应蛋白(hsCRP)在早产儿细菌感染性败血症诊断中的意义。[方法]对2007年10月～2008年8月某院收治的52例疑似细菌感染败血症住院早产儿进行研究,分析hsCRP、传统CRP、外周血常规对早产儿细菌感染性败血症诊断的敏感性、特异性等。[结果]本组细菌感染26例,非细菌感染26例。hsCRP≤3μg/ml29例,3～8μg/ml者23例,敏感度为80.7%,特异度92.3%;传统CRP≤8μg/ml者44例,﹥8mg/dl者8例,敏感度为26.9%,特异度为96.1%;血常规阳性者15例,阴性者37例,敏感度为38.5%,特异度80.8%。hsCRP诊断细菌感染的敏感性高于传统CRP及外周血常规。[结论]在早产儿感染的诊断中,hsCRP升高明显,对诊断感染灵敏,是诊断早产儿细菌感染的适宜指标。[Objective] To evaluate the significance of the high sensitive C reactive protein test for the diagnosis of bacterial infection septicemia of preterm newborns.[Methods] 52 preterm newborn cases with suspected bacterial infection who were admitted to our preterm newborn ward during Sep.2007 to Aug.2008 were enrolled to perform a study.The hsCRP,CRPand blood routine test of all the cases were conducted and analyzed for knowing their sensitivity,specificity,etc.[Results] Among the 52 cases,there were 26 cases with bacterial infection and the other 26 preterm newborns without bacterial infection。 There were 29 cases with hsCRP level below 3 mg /dl（hsCRP≤3 μg /ml） and 23 cases in the range of 3 μg /ml to 8 μg /ml,sensitivity 80.7%,specificity 92.3%,the level of 44 cases was below 8 μg /ml（CRP≤8 μg /ml） and 8 cases above 8 μg /ml（CRP ﹤8 μg /ml）,sensitivity 26.9%,specificity 96.1%.15 cases were with positive results of blood routine test and 37 cases were with negative results of blood routine test,sensitivity 38.5%,specificity 80.8%.The sensitivity of hsCRP for the diagnosis of bacterial infection septicemia was higher than CRP and blood routine test.[Conclusion] The results indicate that the use of hsCRP concentration in preterm newborn can increase accuracy and sensitivity of diagnosis in infection.hsCRP test is feasible for the diagnosis of bacterial infection of preterm newborns.
