红斑狼疮患者ANA、抗ds-DNA和抗ENA多肽抗体的血清学检测

摘　　要：[目的]探讨ANA、抗ds-DNA和抗ENA多肽抗体的血清学检测临床意义和对SLE的诊断价值。[方法]选择2008年1月～2009年12月某院门诊和住院的SLE患者80例为观察组,选择同期在门诊健康体检者60例为对照组。分别采用间接免疫荧光法、胶体金斑点渗透法和免疫印迹法检测两组受检者ANA、抗ds-DNA和抗ENA阳性率。[结果]观察组ANA、抗ds-DNA和抗ENA抗体阳性率明显高于对照组,差异有统计学意义(P﹤0.01);抗ENA抗体谱中Sm、抗SS-A、抗SS-B、抗nRNP、抗rRNP明显高于对照组,差异有统计学意义(P﹤0.01或P﹤0.05)。[结论]ANA、抗ds-DNA和抗ENA在SLE诊断中有互补性,联合检测有利于提高检出率和准确性,对SLE的诊断、疗效评价和预后判定具有重要的临床价值,值得进一步临床推广应用。[Objective] To explore the clinical significance of serological detection of ANA,anti ds-DNA and anti-ENA polypeptide antibodies,and the diagnosis value for SLE.[Methods] 80 patients with SLE in our hospital from January 2008December 2009 were selected as the observation group,60 healthy cases as control group during the same period.Indirect immunofluorescence,colloidal gold dot penetration method and Western blot were used to detect the positive rates of ANA,anti ds-DNA and anti-ENA in two groups.[Results] The positive rate of ANA,anti ds-DNA and anti-ENA antibodies was significantly higher in the observation group than which in the control group,the difference was significant（P﹤0.01）;Sm,antiSS-A,anti SS-B,anti-nRNP,anti-rRNP of anti-ENA antibody spectrum were significantly higher than which in the control group,the difference was significant（P﹤0.01 or P﹤0.05）.[Conclusion] ANA,anti ds-DNA and anti-ENA in the diagnosis of SLE are complementary.Joint testing will help improve the detection rate and accuracy of the diagnosis of SLE.Efficacy evaluation and prognosis have important clinical value,worthy of further clinical spread the applications.
