外科病人并存糖尿病的特点及其围手术期处理

摘　　要：目的明确糖尿病病人围手术期血糖控制的安全方法。方法对６７例外科病人并存糖尿病时围手术期诊断与治疗进行回顾性分析。结果糖尿病以Ⅱ型多见，老年病人占７１６％，病情隐匿，术前漏诊８例，术后因高渗性非酮症昏迷死亡１例，３例发生低血糖昏迷。Objective\ To determine the safe method of perioperative glycemic control in diabetic patients during surgery.Methods\ 67 cases of diabetic patients underwent various surgical procedures were retrospectively analyzed.Results\ The associated diabetes occurred frequently in type Ⅱ elderly patients(71 6%).8 cases were missed diagnosis owing to the insidious diabetic courses,3 cases encountered postoperative hypoglycemic come and 1 case died of hyperosmolar nonketotic coma.Conclusion\ Intravenous regular insulin administration during perioperative period was an effective and safe method for management of the diabetic patients.
