血管内介入技术在中心静脉置管中的应用

摘　　要：目的总结血管内介入技术处理中心静脉置管中疑难护理问题的经验。方法对我院2000年1月～2010年7月13例中心静脉置管患者出现的置管过程不顺、使用中导管移位、导管断裂、血栓形成等护理问题,在DSA下采用血管内介入技术给予相应处理。结果本组13例病例中,导管打折1例,导管移位7例,导管断裂2例,原位换管致导管滑入体内1例,血栓形成2例。除1例股静脉原位换管不慎致导管全部滑入体内未取出,最后行外科处理外,其余异常情况均得到圆满解决。结论中心静脉置管使用过程中疑难护理问题,在影像设备(DSA)的引导下,通过血管内介入技术处理,成功率高,值得推广。Objective To conclude the experience of nursing care problem on center venous catheter implangting by use of intra-vascular interventional technology.Method From January 2000 to July 2010,13 cases of Sichuan Cancer Hospital,who received the center venous catheter implangting,suffered from the nursing care problems such as catheter implangting process difficulty,catheter breakage,catheter displacement or thrombogenesis.We applied the intra-vascular interventional technology with the DSA to solve these nursing care problems smoothly.Result Among 13 cases in the group,1 case＇s catheter folded,7 cases＇ catheter displaced and 2 cases＇ catheter were breakage,1 case＇s catheter slipped into the venous lumen in situ during catheter exchange,2 cases were thrombogenesis.Only 1 case needed surgery treatment because of the catheter slipping into the femoral venous lumen in situ during catheter exchange carelessly,the other problems were solved by use of intra-vascular interventional technology,smoothly and successfully.Conclusion The application of intra-vascular interventional technology under the guidance of the imaging equipment（DSA） in solving the nursing care problems during center venous catheter implangting is feasible and available.
