摘 要:目的:研究我院近4年胆道感染患者的致病菌分布及其药物敏感性,为临床合理用药提供依据.方法:对2007年1月~2010年8月我院收治的287例胆道感染患者胆汁培养的病原菌分布和药敏情况进行统计分析. 结果:287例患者胆汁标本中分离培养出194株细菌, 阳性率67.6%;革兰阴性杆菌126株(64.95%),主要为大肠埃希菌68株(35.05%)、肺炎克雷伯菌26株(13.4%)、铜绿假单胞菌14株(7.22%)等;革兰阳性球菌57株(29.38%),依次为肠球菌30株(15.46%)、葡萄球菌属16株(8.25%)、链球菌5株(2.58%)等;真菌11株(5.67%).革兰阴性杆菌敏感率最高的是亚胺培南;革兰阳性球菌的敏感率最高的是万古霉素.真菌对二性霉素敏感率最高.结论:胆道感染病原体仍以革兰阴性杆菌为主.但革兰阳性球茵、真菌也占有一定比例.根据胆汁培养及药敏结果,合理应用抗生素是治疗胆道感染的关键.Objective:To study the bacteria distribution and drug susceptibility of biliary infection in our hospital during last 4 years, provide the basis for the clinical use of drugs. Methods:287 patients in our hospital biliary tract infection and bacterial culture of bile sensitivity were statistically analyzed , during Jan 2007 and Aug 2010. Results : Bile samples of 287 patients were isolated and cultured 194 bacteria , the positive rate of 67.6%, Gram-negative bacteria 126 (64.95%), mainly EscherichiaColi(68,35.05%), KlebsicllaPneumoniac(26,13.4%), PseudomonasAeruginosa (14, 7.22%), etc. Grampositive cocci(57,29.38%), followed by Enterococci(30,15.46%), Staphylococcus(16,8.25 %), Streptococcus(5,2. 58%), etc. , 11 strains of fungus were tested and occupied 5.67% . Gram-negative bacteria are sensitive to the highest rate of Imipencm; sensitive gram-positive cocci is the highest rate of Vancomycin. Sensitive fungi to Amphotericin highest rate. Conclusions-Biliary tract infection pathogens is still the main gram-negative bacilli. However, gram-positive ball-yan,a certain proportion of fungi. According to the results of bile culture and drug susceptibility, rational use of antibiotics is the key to treatment of biliary tract infection.
